TREATMENT COORDINATOR/Case Manager – FOSTER CARE

UMCF is seeking a Treatment Coordinator to join our treatment foster care team. As a part of a team, the treatment coordinator will support youth and foster families. The treatment foster care licenses foster and adoptive homes, ensures compliance of Ohio and agency rules and policies, facilitates the placement of youth in licensed homes and advocates for placement stability through case management, training and mental health services.

Position Summary

The Treatment Coordinator acts as the main liaison between the foster youth and licensed foster family and the custodial agency. The Treatment Coordinator advocates for all needs of the youth and family including referrals to appropriate services. The Treatment Coordinator facilitates communication with all treatment team providers including other UMCH Family Services team members, custodial agency caseworkers and foster parents to ensure all needs are of the youth and family are met. The Treatment Coordinator provides weekly check-ins with foster families in person and via phone.

Continuing Education

· Paid training in Evidence Based and Trauma Informed Practices is provided

Minimum Qualifications

· Degree in Social Work, Counseling or related field

· Child mental health experience preferred

· Must have strong documentation skills

· Ability to work effectively with families and treatment partners

· Licensure as an LSW/LPC preferred

· Must have a valid driver’s license and be insurable

· Bachelor’s Degree
