Hope Haven East Texas is a Christian, faith-based non-profit providing a direct solution
to the foster care crisis in East Texas. We demonstrate Christs love to children and teens in the foster
care system by providing home, healing, and hope through a combination of residential and family care,
life skills training, and professional mental and behavioral support because we believe that every child
deserves to be safe and equipped for the future.
CASE MANAGER
REPORTS TO: Lead Case Manager
QUALIFICATIONS:
1. Must be 22 years of age or older.
2. Have a masters degree or higher from an accredited college or university, OR
3. Have a bachelors degree in social work or other human services field from an accredited college or university and one year of full-time documented experience in child and family services, OR
4. Have any bachelors degree from an accredited college or university and two years
full-time documented experience in child and family services.
5. Must have a valid Texas drivers license and be eligible for insurance coverage by Hope Havens automobile insurance company.
6. Documented Experience to include the following:
a. Providing support services to youth and young adults who may have experienced child abuse and neglect, are in foster care, are homeless, are at risk or have mental or behavioral health challenges.
b. Providing independent living services to youth and young adults.
Compensation Status: Full-Time Salaried (Exempt)
GENERAL RESPONSIBLITIES AND DUTIES
1. Provide basic case management services, including:
a. Creation of and maintenance of the Childs Service Plan according to a specific GRO Service Package requirement.
b. Maintain all records as required by DFPS for General Residential Operations Minimum Standards AND Residential Contracts in a timely and accurate manner.
c. Maintain all records as required by Single Service Continuum Contractors (SSCCs) in a timely and accurate manner.
2. Facilitate Family Support services for children while in a placement at Hope Haven
3. Establish a client relationship with assigned children and meet with them regularly exceeding regulatory requirements.
4. Monitor and facilitate the childs:
a. Education participation and academic performance.
b. Physical and Mental health care.
c. Normalcy and recreational participation.
2. Participate with other Hope Haven personnel to provide assessments, information, and records about the childs participation in the specific Service Package
3. Anticipate and maintain records necessary to track and report performance objectives, and particularly the childs progress regarding anticipated length of stay.
4. Communicate and respond to DFPS inquiries and meetings.
5. Serve as the liaison between Hope Haven, each childs legal team, and community and school contacts.
6. Identify, develop, and maintain positive relationships with community resources and members of each childs team.
7. Monitor and report on resident compliance with house rules.
8. Participate with Treatment Team in treatment planning and staffing for each child in your case load.
9. Serve as a driver and transport children to various appointments and outings.
10. Attend and participate in staff meetings as required.
11. Participate in all required training and maintain professional development in the field to enhance knowledge and skills.
12. Participate in required training. For this position, 30 hours of job-related training are required for the first year of assignment to child-placing responsibilities. Thereafter, 20 hours of job-related training are required.
13. Perform other assignments and duties as needed.
POSITION REQUIREMENTS, KNOWLEDGE, SKILLS, AND ABILITIES
1. Requires basic understanding of a field of knowledge that involves the ability to define problems, collect data, establish facts, and draw conclusions dealing with abstract variables.
2. Ability to be certified in CPR & First-Aid, Medication Administration and Behavior intervention which includes physical restraint of young adults.
3. Ability to recognize differences in sound, such as voices/noises that are loud and playful instead of angry or combative; and ability to differentiate tones and volume in conversations to determine mood and response to person.
4. Ability to work independently under little supervision.
5. Ability to exercise patience, good judgment, and confidentiality while observing and making accurate assessments of situations with available information.
6. Ability to be on call as assigned and attend training and events after office hours and/or on weekends as needed.
7. Ability to work with frequent interruptions and in stressful situations; ability to respond in crisis situations.
8. Ability to establish and maintain effective working relationships with young adults, other employees, and the public.
9. Ability to work successfully as part of a team to achieve strategic results.
10. Have excellent verbal and written communication skills to effectively communicate the mission and vision of the organization and the needs of children in foster care.
11. Ability to motivate others appropriately and effectively.
12. Must acknowledge the unique Statement of Faith mission of Hope Haven of East Texas and respond appropriately to the cultural differences among the agencys service population.
