About the Role

We are seeking aNurse Practitionerto provideon-site rounding services in nursing homes/skilled nursing facilities, with a primary focus on treating

a combinationof acute and chronic pain syndromes.

This role is ideal for NPs who enjoyindependent practice, valueconsistent schedules, and are comfortable managingcomplex, medically fragile populations.

What Youll Do

Conduct regularon-site roundsin assigned facilities

Evaluate and manageacute and

chronic pain conditions

Prescribe and manage medications, includingcontrolled substances

Coordinate care with facility staff (PTs, RNs, DONs, administrators)

Complete timely EMR documentation

Patient Population

Acute rehabilitation and long-term care residents

Geriatric patients with multiple comorbidities

Chronic pain, mobility-related pain, and dementia-associated pain syndromes

Schedule & Work Environment

Primarilyweekday daytime hours

No hospital shifts

Travel between facilities is required in some cases

Predictable patient census and rounding schedules

What Were Looking For

ActiveNP licensein the practicing state

DEA license(or ability to obtain)

Experience or strong interest in:

Geriatrics

Long-term care

Pain management

Comfortable working independently and traveling locally
