Job Title: Care Coordinator

Location: Johnson County Hospital, Tecumseh, NE

Department: Nursing

Reports To: CNO

Position Summary:

The Care Coordinator is responsible for overseeing and facilitating transitions of care to ensure continuity, quality, and safety for patients across healthcare settings. This role serves as the primary point of contact between patients, hospitals, primary care providers, specialists, and other members of the healthcare team. The Care Coordinator monitors high-risk patients, manages referrals, supports post-discharge planning, and ensures compliance with regulatory and clinical standards.

This position requires strong clinical knowledge, critical thinking skills, and the ability to work independently in a dynamic healthcare environment.

Duties & Responsibilities:

Monitor and manage a high-risk subset of patients following discharge to home.

Conduct post-discharge telephone calls to assess patient status and reinforce discharge instructions.

Schedule follow-up appointments and coordinate necessary services.

Manage referral processes including:

Receiving, reviewing, and processing provider referrals.

Coordinating appointments with specialists and ancillary services.

Ensuring required documentation and medical records are transmitted to receiving providers.

Tracking referral completion and documenting outcomes.

Following up on outstanding referrals to ensure timely patient access to care.

Communicate directly with primary care providers and specialty providers regarding patient status, referrals, and care plans.

Lead transition of care planning and serve as the main point of contact between the patient, hospital, and primary care provider.

Provide education regarding medications, therapies, and ongoing care needs.

Identify barriers that may hinder a patient's ability to meet continuing care needs and collaborate on solutions.

Document all aftercare, referral, and transition-related information in the patient record.

Identify and connect patients with appropriate community resources to improve program effectiveness and patient outcomes.

Monitor referral patterns and identify opportunities to improve care coordination efficiency and patient access.

Perform additional duties as assigned and modified at the discretion of management.

Qualifications & Skills:

High level of proficiency with Microsoft Office Suite, including intermediate Word, Excel, and PowerPoint skills.

Strong interpersonal, written, and verbal communication skills.

Ability to work independently and manage time effectively.

Ability to adapt to evolving regulations, facility needs, patient needs, and provider expectations.

Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Nebraska.

Working knowledge of CMS regulations and healthcare compliance standards.

Knowledge of referral workflows, insurance authorization processes, and care coordination best practices (preferred)

Scope of Work

This position involves electronic medical record review, telephonic patient outreach, referral coordination, and collaboration with interdisciplinary healthcare teams. The role is expected to evolve based on regulatory requirements and organizational needs.
