We are seeking a dynamic, highly skilled and motivated Nurse Practitioner specializing in Family Medicine and Functional Medicine to join our innovative healthcare team. Responsibilities

Conduct thorough patient assessments including vital signs, physical examinations, and detailed health histories to identify underlying causes of chronic conditions and wellness goals.

Develop and implement individualized treatment plans that incorporate functional medicine techniques such as nutritional counseling, lifestyle modifications, hormone therapy, supplements, peptide therapy, etc.

Utilize electronic health record (EHR) system to document clinical findings accurately and ensure seamless continuity of care.

Manage acute and chronic conditions.

Collaborate with multidisciplinary teams aligned with HIPAA standards.

Engage in ongoing clinical education and stay current with emerging trends relevant to functional medicine.

Experience

Proven experience as a Nurse Practitioner with a minimum of 3 years working independently in family medicine/functional medicine practice.

Extensive background in functional medicine practices including nutrition counseling, metabolic assessments, and integrative therapies, hormone replacement therapy.

Demonstrated proficiency with EMR/EHR systems.

Ability to work effectively with diverse patient populations.

Excellent assessment skills combined with physiology knowledge for diagnostic evaluation across internal medicine disciplines such as gastroenterology, cardiology, and endocrinology.
