The Program Coordinator is responsible for caseloads of consumers specific to monitoring and supervising their programs, care, associated staff, and independent contractors.
Supervisory Responsibilities:
Supervises and trains Direct Support Professionals, including but not limited to, providing initial orientation and trainings, assistance in meeting training requirements, bimonthly day program visits, and bimonthly supervisions. Essential Duties/Responsibilities:
Supports and teaches, and monitors contract compliance of independent contractors, including, but not limited to, conducting bi-monthly site visits to the clients home and weekly phone contact.
Provides a positive, supportive role-model for the consumer.
Teaches and reinforces appropriate consumer behavior to consumer, home provider, and direct support professionals through direct feedback, role-playing, and behavior modification techniques.
Upholds client right in a manner that supports an individuals choice and their right to self-advocacy
As a role model, maintains professional boundaries with colleagues, team members, individuals and the provider community.
Documentation and timely submission of all progress notes, weekly summaries, attendance, and all other consumer documentation deemed necessary by the company, including the maintenance of client records.
Coordination of clinical supports including medical, therapy, and related appointments.
Communicates with contracting agencies specific to program development, on-going monitoring and modifications, and documentation and reporting requirements.
Participation in annual Individual Service Plan meetings.
Liaison between guardian and agency specific to program explanation, updates, paperwork requirements, and addressing guardian concerns.
Reports to, and provides regular updates to Program Manager specific to caseload.
Develops, implements and coordinates use of protocols for program activities; including menu planning/grocery lists, menu and budget planning, along with other activities of daily living.
Obtains and coordinates resources for community activities including social recreation opportunities, vacation, vocational, and volunteer activities, etc. in conjunction with other staff and independent contractors.
Develops, maintains, schedules, and monitors respite for specific consumers, including orientation of respite providers.
Identifies and addresses potential safety issues for specific consumers.
Participation in on-call rotation schedule.
Required Skills/Abilities:
Excellent verbal and written communication skills.
Strong interpersonal and negotiation skills
Required Education and Experience:
BA/BS or higher in Human Services or related field,
1-2 years of experience in Human Services or related field.
Travel requirements:
Weekly travel - Services to be performed in-home settings
