Certified Nurse Practitioner

Preferred Healthcare delivers compassionate, high-quality primary care for homebound seniors across the Cincinnati area.

We are seeking a dedicated and experienced Nurse Practitioner to join our team and provide in-person care in residential homes, assisted living communities, and long-term/post-acute care facilities.

Key Responsibilities:

Deliver comprehensive primary care services to a geriatric population with complex, chronic conditions

Coordinate care with interdisciplinary teams across various settings

Maintain accurate electronic documentation and adhere to best practices in clinical care

Foster meaningful relationships with patients, caregivers, and facility staff

Qualifications:

Active Nurse Practitioner license in the state of Ohio

1 year of experience (preferred) in geriatrics or chronic disease management

Strong clinical decision-making and assessment skills

Proficient in electronic health records and digital tools

Excellent communication and interpersonal skills
