About the Role:

The Licensed Practical Nurse (LPN) plays a critical role in delivering high-quality patient care within the long-term care settings. This position involves providing direct nursing care under the supervision of registered nurses and physicians, ensuring patient comfort, safety, and well-being. The LPN will be responsible for monitoring patient health, administering medications, and assisting with daily living activities, contributing to comprehensive care plans. This role requires strong clinical skills combined with compassionate communication to support patients and their families effectively. Ultimately, the LPN helps to maintain a safe and therapeutic environment that promotes positive health outcomes and patient satisfaction.

Minimum Qualifications:

Current and valid Licensed Practical Nurse (LPN) license in the state of Minnesota.

Completion of an accredited practical nursing program.

Basic Life Support (BLS) certification.

Demonstrated knowledge of nursing principles, patient care techniques, and medication administration.

Strong communication and interpersonal skills to interact effectively with patients and healthcare team members.

Preferred Qualifications:

Experience working in a clinical or long-term care setting within Minnesota.

Familiarity with electronic health record (EHR) systems.

Responsibilities:

Provide basic nursing care to patients, including monitoring vital signs and reporting changes to the healthcare team.

Administer prescribed medications and treatments accurately and safely according to established protocols.

Assist patients with activities of daily living such as bathing, dressing, and mobility to promote independence and comfort.

Document patient information, care provided, and observations accurately in medical records to ensure continuity of care.

Collaborate with registered nurses, physicians, and other healthcare professionals to implement and update individualized care plans.

Educate patients and their families on health maintenance, disease prevention, and post-treatment care instructions.

Maintain a clean, safe, and organized work environment in compliance with healthcare regulations and standards.
