Position Summary:
The Case Manager is a vital member of the multidisciplinary team, responsible for delivering high-quality, client-centered case management services within our Partial Hospitalization Program (PHP), Intensive Outpatient Program (IOP), and Outpatient Program (OP). This role focuses on providing holistic, community-based care to individuals with co-occurring mental health and substance use disorders, ensuring that each client receives comprehensive support that promotes recovery, wellness, and long-term stability.
Key Responsibilities:
Client-Centered Support:
Conduct comprehensive needs assessments and develop individualized service plans in collaboration with clients, families, and the treatment team.
Advocates for client needs and preferences, ensuring their voice is central in all decision-making processes.
Holistic Care Coordination:
Link clients to appropriate community resources, including housing, vocational support, healthcare, peer support groups, and social services.
Coordinate care with mental health, medical, and substance use providers to ensure seamless integration of services.
Treatment Planning & Documentation:
Participate in treatment team meetings to review progress, update goals, and modify plans as needed.
Maintain accurate, timely, and confidential documentation in accordance with program policies and regulatory standards.
Community Integration:
Foster relationships with local organizations, agencies, and support networks to expand available resources for clients.
Promote community engagement opportunities that support recovery, personal growth, and social connectedness.
Crisis Support & Intervention:
Provide crisis intervention, de-escalation, and support during periods of acute need.
Collaborate with the clinical team to ensure appropriate follow-up care and safety planning.
Qualifications:
Knowledge of co-occurring mental health and substance use disorders, recovery-oriented systems of care, and trauma-informed practices.
Strong communication, organizational, and problem-solving skills.
Ability to work collaboratively in a multidisciplinary team and independently in the community.
Familiarity with local resources, benefits systems, and community supports.
Valid driver’s license, reliable transportation, and willingness to travel within the community.Core Competencies:
