Key Responsibilities:

Administer medications and treatments as prescribed by physicians.

Monitor and assess patients' health status, documenting changes appropriately.

Assist residents with activities of daily living and ensure their comfort and safety.

Collaborate with the interdisciplinary healthcare team to implement effective care plans.

Maintain compliance with facility policies, state and federal regulations.

Provide emotional support and education to patients and their families.

Requirements

Current and valid NY State LPN license.

Minimum one year of experience in a long-term care or similar healthcare setting.

Strong communication, assessment, and organizational skills.

Ability to work efficiently both independently and as part of a healthcare team.

Current Basic Life Support (BLS) certification.
