Heartline Staffing, Strategic Talent Partners with Purple Cow Recruiting, is seeking a Registered Nurse Case Manager in Clewiston, Florida.

This is a full time outpatient Registered Nurse Case Manager opportunity focused on coordinating and managing patient care across complex outpatient and discharge planning needs. The role supports patients, families, and interdisciplinary teams to ensure quality outcomes, appropriate utilization of services, and continuity of care.

Core duties include developing and evaluating individualized patient care plans, coordinating discharge planning, performing prospective, concurrent, and retrospective clinical reviews, and serving as a patient and family advocate throughout care transitions. The role may also include in home or hospital evaluations as needed.

Schedule

Monday through Friday 8:00am to 5:00pm

On call rotation approximately one in every six to seven weeks

Average daily caseload of eight to thirteen patients

Compensation:

Annual salary range of 75000 to 95000 based on experience

Monthly fuel stipend of 600 if forty miles or more from the facility

Required

- Associate’s Degree in Nursing

- Active Registered Nurse license in the State of Florida

- Minimum of two years of experience as a Medical Case Manager

- Current BLS certification

Additional

- Case Management certification or ability to obtain within one year of employment

- Ability to manage outpatient caseloads and complex discharge planning needs

- Ability to participate in on call rotation

- Willingness to perform in home or hospital evaluations when required
