Astria Health is seeking a Transition Coordinator to join our team! This position is located at our Toppenish Hospital in the Behavioral Health Unit.

Shift: 8:00am - 4:30pm, Monday - Friday

What you get to do every day!

The Transition Coordinator plays a crucial role in ensuring smooth transitions for patients exiting inpatient behavioral health facilities. You will:

Develop comprehensive discharge plans tailored to each patient's needs, considering factors such as housing, employment, follow-up care, medication management, and social support.

Serve as an advocate for patients to ensure their needs are addressed and rights are respected throughout the discharge process.

Coordinate with inpatient healthcare teams, outpatient providers, social workers, case managers, and community organizations to facilitate seamless transitions for patients.

Identify and connect patients with community resources, including mental health clinics, support groups, vocational training programs, housing assistance, and financial aid.

Provide patients and their families with education about mental health conditions, treatment options, medication management, and strategies for coping with symptoms post-discharge.

Maintain accurate and up-to-date records of discharge plans, referrals, and communication with patients and healthcare providers.

Conduct follow-up assessments to monitor patients' progress post-discharge and address any barriers to successful community integration.

The person in this position must be able to participate in policy approved physical and mechanical restraint as trained with potentially dysregulated and/or combative patients and maintain personal, patient and staff safety without physical restrictions.

Requirements

Current WA State Agency Affiliated Counselor Registration

Bachelor's degree in social work, psychology, counseling, nursing, or a related field OR a combination of education and a minimum of two years experience working in a behavioral health setting

Must maintain current BLS certification
