LPN- Long Term Care

Job Description

PAY: $32-$35/hr

Key Responsibilities

Administer medications and treatments as prescribed

Monitor residents’ vital signs and overall health status

Assess and document changes in condition

Provide wound care and other skilled nursing procedures

Assist residents with activities of daily living (ADLs) when needed

Develop, implement, and update care plans in collaboration with the healthcare team

Supervise Certified Nursing Assistants (CNAs) and delegate tasks appropriately

Communicate with physicians, families, and interdisciplinary team members

Maintain accurate and timely documentation in medical records

Ensure compliance with state and federal regulations

Requirements

Current Licensed Practical Nurse (LPN) license in good standing

Graduate of an accredited practical nursing program

CPR/BLS certification

Knowledge of geriatric care and chronic disease management

Strong clinical assessment and critical thinking skill

4 days a week working evening shift (2p-10p)
