Large Texas Based Geriatric Primary Care Group seeks 5 Nurse Practitioners/NPs or Physician Assistants/PAs in 5 different primary care settings in the Corpus Christi, Texas area!

Requirements:

•Not open to new grads

About the geriatric Primary Care/internal medicine clinic position – outpatient clinic:

•NP or PA will see patients in the primary care clinic – possibly 2 clinics close by each other

•8a-5p, Mon-fri, no weekends, no call, 15 patients a day seen. Quality visits versus quantity mentality.

•Team approach, discussing patients amongst a team on a regular basis. Structured environment.

•will work in a primary care clinic for ages 50 and up.

About the Transition Care Position:

•Transition Care NP/PA- working with patients that are transitioning from the hospital to the home or SNF, they will be seeing the patients either in a home environment or SNF environment but could also see them in a clinic depending on if they are mobile enough. At the clinic, they will have “mini visits” where they are making sure they are transitioning okay, taking meds, checking wounds/incisions, etc.

•Most of the patients will be in the home so the person has to be okay with working independently, on a one on one basis, NOT a new grad.

•There is mileage reimbursement since there will be travel throughout the entire market.

•Most likely be seeing the same patient(s) for several weeks. Hours are the same, M-F 8-5, pay upper 90s-lower 100s.

About the Preventive/Quality Assessments position:

•NP or PA will work in 1 clinic for 1 week or 2 months, then move on to the next clinic until all the patients tests and assessments are up to date. No prescribing needed.

•8a-5p, Mon-fri, no weekends, no call, 8 patients a day seen. NP or PA can work close to home in clinics as close to them as possible.

•Assessments could be for mammograms, immunizations, routine checkups, health risk assessments, patient counseling/teaching, scheduling a colonoscopy, chest X-ray’s, urinalysis, blood pressure check, diabetes check, chronic illness, etc

•MSN/ APRN (Advanced Practice Registered Nurse)/ARNP / FNP, GNP, ANP, ACNP, NP-BC, Family, Adult, Acute Care or Gerontology Nurse Practitioner or Physician Assistant/PA-C will conduct exams and tests for patients ages 50 and up. May refer patients back to PCP when needed.

•Ideal candidate has experience with seniors, experience with diagnosis and assessment of chronic illness, must be able to establish relationships, be personable, flexible and adaptable.

About the Skilled Nursing Facility position:

•skilled nursing facilities, rounding on patients 2-3 times a week, meeting with families/care team as needed, discuss cases with the PCPs.

•Our goal is to decrease length of stay at these facilities.

•The physicians on the hospital team would be available for backup and would still round on the patients as well, just not as often. Hours are flexible but generally M-F 8-5

About the Diabetes position:

•We prefer someone with diabetes experience, preferably with geriatrics or adults.

•Same hours, the NP or PA will be leading the diabetes clinic and at times going to other clinics to see the diabetes patients.

•pay is upper 90s to low 100s

Compensation:

•$90-105,000/year annual salary depending on experience.
