We are seeking a Patient Advocate to join our team!

Rocking Horse Community Health Center (RHCHC) is a Federally Qualified Health Center (FQHC) that provides healthcare services to some of the area's most vulnerable populations.

The organization is looking for an individual to join the team in our Patient Advocacy department. Patient Advocates are responsible for creating and maintaining strong relationships with the patient community and staff to alleviate social and environmental factors that have a negative effect on health. Connects patients with community resources that generally improve utilization of services that support a healthy family function. Works with all patients to ensure timely scheduling, compliance and reduction of barriers to appointments.

Position Info:

Position Title: Patient Advocate

Primary Service Center: Springfield, Ohio (South Limestone Location)

FLSA Status: Non-Exempt - Hourly position

Work Hours: Full-time position

Education Requirements:

High school diploma or (GED) required

Ability and willingness to research available local resources for patients as needed.

Ability to work in a team environment and maintain good working relationships with co-workers and patients.

Ability to work under pressure in a fast-paced environment as required.

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactory. The requirements listed below are representative of the knowledge, skill, and/or ability required. The qualified candidate will be a trusted member of and/or have an unusually close understanding of the community served.

Education and/or Experience: High School diploma or general education degree (GED).

Language Skills: Ability to read and comprehend simple instructions, short correspondence and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients, customers and other employees of the organization.

Mathematical Skills: Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to compute rate, ratio and percent and to draw and interpret bar graphs.

Reasoning Ability: Ability to apply common sense understanding to carry out detailed but uninvolved written oral instructions. Ability to deal with problems involving a few concrete variable sin standardized situations.

Computer skills: To perform this job successfully, an individual should have knowledge of Database software; Internet software; Spreadsheet software and Word Processing software.

Certificates, Licenses, Registrations: A current driver's license and an acceptable driving record. An individual whom poses a direct threat to the health and safety of himself/herself or others in the workplace will be deemed not qualified for this position.

Excellent interpersonal communication, organization, and managerial skills are also necessary.
