Job Description

This is a hybrid role that includes fieldwork and remote responsibilities and requires a minimum of 2 in-office days per week for collaboration and case coordination.
Key Responsibilities
Manage and oversee assigned members’ care plans
Conduct monthly phone contacts and quarterly in-person visits
Advocate for members’ clinical and service needs
Coordinate communication between members and providers
Monitor compliance with approved service plans
Recommend care plan modifications as needed
Maintain thorough and accurate documentation
Participate in collaborative weekly team meetings
Requirements
Bachelor’s Degree (Social Work, Human Services, or related field preferred)
2+ years of experience in case management, healthcare, or human services
Strong organizational skills and ability to manage multiple priorities
Comfortable with field work, documentation, and a hybrid work environment
Valid driver’s license and reliable transportation
Bilingual candidates strongly encouraged to apply
