About the Job

Overview:

This position is assigned to the Local Reentry Council in the specified North Carolina county. The Community Case Manager holds a caseload of individuals returning to the community from prison, jail, probation, or parole, as well as those with a criminal history who require support services. The goal is to facilitate a successful reintegration by assessing needs and risks, coordinating services, providing guidance, and ensuring continuity of care pre- and post-release.

Key Responsibilities:

Clearly communicate client expectations and Council rules.

Ensure clients complete assessments, treatment, or required classes; monitor and verify participation.

Collaborate with community partners to reinforce pro-social behavior.

Build a therapeutic professional alliance with clients.

Address noncompliance or risky behaviors during interactions and provide corrective guidance.

Use validated risk assessments to prioritize high-risk needs for intervention.

Link clients to community resources and schedule follow-up appointments.

Facilitate goal setting and target development in coordination with clients and service providers.

Monitor client progress and adjust plans as needed.

Maintain accurate records of client data and services via the NC DPS CART (Community Automated Reentry Tool) system. Minimum Qualifications:

Associate degree (2-year) in Criminal Justice, Human Services, Recreation, or a related field; and at least two years of case management or human services experience; or an equivalent combination of education and experience.
