Program & Location: Family Preservation Program - New Bedford, MA
Education & Experience: Associates Degree or higher preferred; minimum of High School Diploma or GED with at least one year of related job experience.
Pay Range: $20-$25/hr (education & experience dependent)
Status/Schedule: Part Time, 20 hours per week

SEMCOA's (Southeast Massachusetts Council on Advocacy) mission is to advocate for and provide support to individuals, families, and communities in the areas of substance use, mental illness, domestic violence, homelessness, and parenting. SEMCOA operates four emergency assistance congregate family shelters and additional scattered sites that provide supportive services to families and assist them with securing permanent housing. SEMCOA also provides residential recovery home treatment to individuals with substance use disorders, as well as transitional and permanent housing stability and support programs. Our portfolio also includes two graduate sober houses, the Family Preservation Program, as well as Greater Brockton and Greater Attleboro Taunton Healthy Families programs.
Case Manager Requirements
Ability to work with families from diverse social, economic, ethnic, cultural and racial backgrounds
Basic understanding of Substance Abuse/Mental Health co-morbidity diagnosis
Assessment and case management skills
Knowledge of services necessary and resources available to assist homeless individuals recovering from Substance Use/Disorders
Possess high professional ethics and ability to maintain healthy boundaries
Excellent verbal and written communication skills
Knowledge of basic computer skills and software including Microsoft Word/Excel
Ability to work independently without direct supervision while adhering to the program's philosophical team approach
TB screening
Certification in the HPAO approved de-escalation program
Compliance with Federal Regulations, 42 CFR Part 2, Confidentiality of Alcohol and Drug CPR/First Aid Certification Case Manager Duties & Responsibilities
Compliance with regulating agencies (i.e., BSAS, HUD, etc.)
Maintain inspection documentation on all units, communicating with landlords as appropriate
Scheduled on site meetings with families and enforcement of program rules and policies
Understand and follow established referral process and schedule intakes
Facilitate intakes of new residents including setting up new record of completion of intake paperwork
Completion of Initial Individual Service Plan (ISP) with resident
Referrals for needed services to appropriate agencies/providers including but not limited to: DTA, SSA, DSS, Medical/Mental Health agencies, etc.
Maintain accurate case files, ensures required Authorizations to release/obtain information are signed by residents, prepares legible case management notes of each session, documents referrals on designated form, updates ISP's monthly and ensures periodic evaluations are done in a timely manner
Meet with each resident as scheduled to review/evaluate progress in treatment/recovery
Revise/update the resident's Individual Service Plan as indicated, including the establishing of additional goals/objectives and the steps to achieve these goals/objectives
Ensure timely completion of Admission/Discharge satisfaction surveys by residents
Facilitate/Co-facilitate workshops with residents
Attend monthly case review with Program Director
Attend clinical supervision, required staff meeting and trainings
Other duties as assigned by the Program Director. Case Manager Qualifications
