Intake & Staffing Coordinator March 2011 - present

Receive all new referrals and process them for eligibility and authorization

Staff all patients with respective personnel (nursing, therapy, social worker, etc.)

Receive frequency and change of patient status reports

Coordinate with physician and agency Director and/or Manager regarding any changes in patient condition

Maintain and prepare weekly, monthly and quarterly reports

Account for and obtain compliance of submission of documentation by field personnel (visit notes, evaluations, communications, reports, HR requirements, etc.)

File & organize all patient charts
