Patricia Zimmerman has been a Policy Associate/Program Developer, Public Speaker, Mentor, Advocate, Case Manager, Consultant and now supervisor to those within the specialized communities that she serves. Ms. Zimmerman's expertise is in advocacy and her recent focus has been on supporting families and not just the individual in the transition from prison to the community. This work has given her the unique opportunity to assist in areas of mental health, substance abuse, HIV/AIDS, medical adherence, benefits entitlements, housing, prison reentry, family reunification, criminal justice involvement and more. Senior Case Manager
Participates in the intake process for all new clients (homeless men in an on site working shelter) by completing an assessment that covers everything from family history, mental health, level of education to work history. Develops and facilitates support groups within the program for clients. Provides supportive counseling and 24 hr crisis intervention around a host of issues integral to living with mental illness/substance abuse. Assigns cases to members of a clinical team based on caseload and review of service plans to ensure program and contract compliance. Completes psycho-social evaluations for all clients based on need. Serves as primary client contact to foster participation of clients' family and significant others. Documents client contacts and maintains a confidential record. Policy Associate Supervisor
Responsible for assisting families (broadly defined) address issues of policy reform directly related to various aspects of the criminal justice system and other government agencies. I conducted program planning and development for a new 'storefront' in the East New York/Brownsville sections of Brooklyn. A strengths-based, family focused approach was used to assist individuals and families with issues surrounding criminal justice involvement. I helped develop family related strategies to address issues in their lives (Substance Abuse, Medical/Mental illness, Domestic Violence, etc.), with a particular focus on the co-occurring factor of HIV/AIDS.

Responsible for managing a team of four case managers, with my own caseload of 40 men and woman; along with their identified family members. This was an exclusive department that only accepted individuals for services who were HIV+ and CDC Defined. I have been trained on Hippa and Confidentiality Laws. Intensive Case Management services were conducted; which included but was not limited to medical advocacy, assistance with benefit entitlements, family reunification, etc.
