MAIN FUNCTION, work under collaboration with the Physician, provides primary and acute services to Residents as a member of a health care team in a primary care extender/ care management role.

RESPONSIBILITIES include:

1. Identifies clinical care issues, providing direct primary care as an extension of the PCP for short term, sub acute and ill residents at Mary Manning Walsh Home.

2. Performs full physical assessments to all new admissions and acutely ill Residents.

3. Performs health assessments, orders or performs diagnostic tests, interventions and skilled services within the Nurse Practitioner‘s scope of practice. Makes rounds of all short term/sub acute Residents and documents pertinent findings in the electronic medical record.

4. Works closely with attending physicians/Nursing Staff as a member of the health care team to collaborate on treatment plans.

5. Collaborates with facility Medical Director/Physicians and communicates issues and concerns regarding the medical care of the residents.

6. Interacts with residents’ families to keep them abreast of the Resident’s condition.

7. Collaborates with the interdisciplinary team in the Residents discharge process.

. Serve as primary RN on a 34- bed Medical-Surgical/geriatric unit with Step-down/ Telemetry and Neurology units), caring for patients with post-catch, CHF, and, COPD, diabetes, ESRD, AIDS, TB/infections disease, orthopedic, neurological/seizure disorder, psychiatric, GU-GYN, and other medical-surgical conditions

Manage and provide ventilators, tracheotomies, IVs, central lines cares, titrated cardiac medications, arrest codes, COPD, monitoring, PT/OT support, tube feeding, TPN, chest tubes, PCA/pain management, ostomy/wound care, pressure ulcer prevention/care, isolation, and extensive patient-family teaching
