Job Description:

I was performing holistic nursing care to the patients. Admissions and Discharge of patients, assessment, and evaluation of vital and ensuring strict compliance with early warning signs to prevent deterioration of patients, patient and family education. Applying SBAR in interdisciplinary communication across the continuum of care. Management of children with various conditions requiring that require long term care.

Airway management: Suctioning, nebulization, chest physiotherapy, assisting in insertion or tracheostomy tube and care after insertion, managing mechanically ventilated patient and monitoring them for deterioration and possible weaning.

Exposure: Temperature monitoring and if need be, targeted temperature management especially in case of post-cardiac arrest patients.

Gastro-Intestinal: Management of patients on Colostomy, Percutaneous, Endoscopic Gastrostomy, Percutaneous, Endoscopic Jejunostomy requiring other forms of nutritional support other than oral feeding. Nasogastric tube insertion and monitoring, enema insertion.

Job Objectives:

Ensure quality implementation of delivery of SMC services at the community level by HFW/supervisors and CHWs through the timely delivery of SMC commodities and supplies.
