As a flex provider, perform health assessments for Medicaid members within their home living environment. Identify quality improvement activities, unmet medical needs, gaps in care, initiate appropriate referrals to health plans. May 2017- July 2018 Guardian Healthcare Providers, Louisville Kentucky Advanced Practice Nurse

Intermediate care facility provides 24 hour services to 100 developmentally disabled individuals living at the facility and three local homes. Under direction of the Medical Director, responsibilities include primary care for a caseload of 24 individuals with medically complex developmental disabilities. This includes seizure management, management of behavioral problems, motor dysfunction and various secondary medical conditions such as pneumonia, GERD, and osteoporosis.

2012 to 2014 Division of Medical Assistance, Hamilton Twp., NJ Advanced Practice Nurse

Oversight of New Jersey Medicaid contract, consumer complaints and interfacing with HMO’s and ensuring

individual’s medical services were provided.

2011 to 2012 Hunterdon Developmental Center, Clinton, NJ Advanced Practice Nurse

Intermediate care facility provides 24 hour services to 540 developmentally disabled individuals living at the facility. Under direction of the Medical Director, responsibilities include, primary care for a caseload of sixty-eight individuals with medically complex developmental disabilities. This includes seizure management, management of complex behavioral problems, motor dysfunction and various secondary medical conditions such as pneumonia, GERD, and osteoporosis. Additional responsibilities include all new admission evaluations and management of respite program. Chairperson of the Medical Human Rights Committee.

2005 to 2011 Consortium on Innovative Practices Adult Nurse Practitioner Consultant

As consultant conducted fundamental surveys in various states which included but was not limited to, review

of sample/focus selection of individuals for facility strengths, needs and assessment of effective services.

Review and serve as a resource for new developments and enhancement of policies and procedures. Prepared

numerous ICF facilities for partnership surveys for ICF/CMS. Expert consultant (Nursing/Medical) for DDS

Legal Division for California facilities under U.S. Department of Justice (CRIPA) Investigations. Developed

statewide best practice guidelines for nursing services for the state of California.

2004 to 2007 Warren Haven Care Center Per-Diem Charge Nurse

Provide float nursing services during the 11-7 tour as needed, to supplement the facility full-time staff.

Long term care included supervision of nursing assistants, utilization of the nursing process to ensure all

patients are properly cared for and all deviations from the norm are addressed. Constantly aware meet needs

of patients to assist with independence.

Participated with medical administration in formulating policy pertaining to nursing services required by developmentally disabled individuals. Provided nursing staff with inservice education pertinent to nursing service to developmentally disabled individuals on a continuing basis. Recruited, interview, and recommended employment to the nursing department. Prepared statistical reports of the nursing section containing findings, conclusions and recommendations. Investigated unusual incidents and issued reports of findings. Developed policy and procedures for infection control.

1995 to 1998 Hunterdon Developmental Center, Clinton, NJ Supervisor of Nursing

Acting as a nurse Officer of the Day, was in charge of the general nursing care of 650 individuals at this intermediate care facility. Oversaw professional nurses and other employees of the nursing department and supervised their work. Supervised head nurses and licensed practical nurses and took measures needed to insure that the physicians’ orders were carried out and that the patients received proper care. Evaluated their work performance. Prepared accurate, professional detailed reports. Supervised the Out-Patient Department.

1993 to 1995 Hunterdon Developmental Center, Clinton, NJ Head Nurse

Supervised the working of a twenty-bed infirmary, providing acute care for the developmentally disabled. Developed acute nursing care plans. Developed medication awareness training for patients; provided individual training at each medication administration. Assured that infection control issues were addressed and reported. Presented clinical nursing information to staff as continuing education. Formally, provided ongoing direct care inservice on medical/nursing issues monthly, and informally as needed.

1990-1993 Hunterdon Developmental Center, Clinton, NJ Staff Nurse

Assisted the head nurse as a member of the treatment team. Carried out therapeutic and medical orders issued by physicians. Assisted physicians with examinations of individuals. Provided bedside nursing care. Prepared and maintained daily medical records of individuals of medication and treatments ordered and administered. Maintained record of narcotics on hand and administered. Assumed responsibility of the unit in the absence of the head nurse. Reviewed and maintained the appropriate standards and level of care for individual needs. Assisted the head nurse in writing IHP evaluations on assigned individuals.

1985 to 1990 Hunterdon Developmental Center, Clinton, NJ Licensed Practical Nurse

Under the supervision of the head nurse, administered medications and treatments to individuals. Assisted in the physical examination of individuals. Charted individuals’ treatments and medications under physician’s orders. Collected specimens from individuals and prepared such for the laboratory. Evaluated the positive and/or negative effects of medications on individuals.
