My name is Carlos O. Esquivel, M.D., Ph.D., and I am the Chief of the Division of Abdominal Transplantation in the Stanford Department of Surgery, Co-Director and Associate Director of the Institute for Immunity, Transplantation and Infection, and a Professor of Surgery. I am writing this letter in support of Dr. Giovanni Ambrosino’s O-1 Extraordinary Ability visa application. Based on my knowledge of Dr. Ambrosino’s achievements to date, I am confident that he absolutely qualifies for this highly restrictive category. As a world-renowned adult and pediatric transplant surgeon, I launched my research career at the University of Lund in Sweden, and presented research throughout my career at the most renowned and highly-selective medical institutions in the world. I am very familiar with celebrated professionals in my field all over the globe, and Dr. Giovanni Ambrosino is one of these extraordinary professionals.

I have known Dr. Ambrosino for my entire career since I was a fellow at the University of Pittsburgh under transplant pioneer Prof. Thomas E. Starzl, M.D., Ph.D. I was appointed as Assistant Professor of Surgery in 1985 when Dr. Ambrosino joined the T. Starzl Transplantation Institute. Since then, we have remained international colleagues, and he is, in my expert opinion, one of the world’s foremost clinical, transplantation and oncology surgeons. Dr. Ambrosino is specifically known for his unparalleled expertise with bioartificial liver transplants, hepatocytes transplantation, domino liver transplants, and robotic stereotactic surgery. Dr. Ambrosino was a visiting scholar for several months in 1991 to the multi-organ transplant program at California Pacific Medical Center (Former Pacific Presbyterian Medical Center), whereby he shared his experience of developing innovative research programs in Italy. In his areas of expertise noted above, Dr. Ambrosino has achieved numerous internationally- celebrated and major original contributions of great significance to the medical field, particularly liver transplantation and treatment. Dr. Ambrosino pioneered the infusion of liver cells into patients diagnosed with a rare disorder affecting the liver to produce an important enzyme. This technique was developed to treat Crigler-Najjar syndrome in a painless way, avoiding liver 750 Welch Road Suite 319, Palo Alto, CA 94304 650-***-**** adsi6d@r.postjobfree.com transplant. He has made very important contributions in the multidisciplinary treatment of liver and pancreas cancer.

Dr. Ambrosino is a superb speaker. He has been invited to many educational programs sponsored by scientific societies as well as participating in educational programs on television and other digital platforms such as webinars, YouTube among others. Dr. Ambrosino is also a talented writer. He has authored numerous publications in peer review journals. But, his talents transcend medicine. He was the main author for a screenplay which won the FIRST WINNER PRIZE – Hollywood International Moving Picture Film Festival –Screenplay section in 2016. Dr. Ambrosino has clearly made a significant impact on the medical field and beyond through his outstanding work in liver transplantation, research, and treatment. I believe his work at major hospitals throughout Italy and abroad, as well as his membership in multiple prestigious medical organizations all prove his extraordinary ability as a surgeon and speak to the great benefit he would have if allowed to continue his work and research in the United States. Sincerely,

Carlos O. Esquivel, M.D., Ph.D.

To Whom It May Concern:

My name is C. Richard Piazza, PhD, and I am Executive Chairman of IGL Pharma Inc, a U.S. Biotech Company. I am writing this letter in support of Dr. Giovanni Ambrosino’s O-1 Extraordinary Ability visa application. Based on my knowledge of Dr. Ambrosino’s achievements to date, I am confident that he absolutely qualifies for this highly restrictive category.

In my position as President & CEO of ViatGen, Inc.(1993-2006), I had the pleasure of working with Dr. Giovanni Ambrosino, a truly extraordinary surgeon who was employed in a critical and essential capacity (Senior Clinical Advisor and Member, Medical Advisory Board) at VitaGen, Inc. from approximately 1997-2005. At ViatGen, Inc. we engaged Dr. Ambrosino because of his internationally-regarded skills as a highly- respected clinical, transplant and oncology surgeon and he was a great benefit to our overall operations. Specifically, Dr. Ambrosino made major contributions to our development of the world’s first bioartifical liver and his input in the design and clinical application was a critical part of our development program. Dr. Ambrosino’s knowledge and skill as a both a surgeon and clinical scientist provided a level of expertise that I felt was instrumental in our clinical advancement of the artificial liver. Further, Dr. Ambrosino had a “world view” in providing the background necessary to guide us in the development of our critical clinical trial. His insight and knowledge were invaluable to us. His expertise in assessing and treating acute liver failure provided us with the framework to determine the clinical utility and application of the artificial liver. He is a truly groundbreaking pioneer in his field and brought VitaGen, Inc. significant international visibility and renown through his incredible medical achievements and contributions. Clearly, Dr. Ambrosino’s work has been incredibly crucial to ViatGen’s mission of providing the best health outcomes for patients, as well as enhancing the company’s reputation as a pioneer of ground-breaking technologies. Additionally, it is important to note that Dr. Ambrosino made numerous original medical and scientific contributions of major significance during his time as a clinical advisor to Vitgen, Notably, Dr. Ambrosino’s helped create true firsts in determining the use of the artificial liver in keeping patients alive and even recovering their own liver function thus avoiding a transplant. It was Dr. Ambrosino’s insight that helped to expand our applications from simply a transplant bridge (i.e. keeping a patient in liver failure alive until a transplant was available) to extending its use allowing patients the opportunity to regain native liver function. Perhaps most significant was his insight in how to use the artificial liver in a fashion that allowed oncology surgeons to be more surgically aggressive in liver resection (i.e. tumor removal) that would help achieve better outcomes. Dr. Ambrosino’s tenure with us would qualify as both “original” and of

“major significance” to the field of liver treatment. This description should not only describe the contribution itself, but the impact the contribution has had in the field of medicine. There can be no doubt that Dr. Ambrosino’s contributions have had a significant impact on the field of medicine, not just in the U.S. but internationally. The contributions Dr. Ambrosino has made to the field of medicine are remarkable and absolutely qualify him for a visa in the United States. The methods and surgical techniques that he developed have ground-breaking implications in the medical community and I believe he is an invaluable asset who will continue to carry modern medicine forward in the coming years. Thus, I urge the expedient approval of his visa petition. If I can be of any further assistance, or provide you with any further information, please do not hesitate to contact me.

My name is Andrei C Stieber, M.D, F.A.C.S., and I am a recently retired transplant and HPB surgeon. I am writing this letter in support of Dr. Giovanni Ambrosino’s O-1 Extraordinary Ability visa application. Based on my knowledge of Dr. Ambrosino’s achievements to date, I am confident that he absolutely qualifies for this highly restrictive category. As a respected transplant surgeon in the field of organ transplantation, I have worked both at the University of Pittsburgh on the team of dr. Thomas E. Starzl, the father of liver transplantation, from May 1985 to August 1992 and at Emory University in Atlanta as the co-director of the Liver Transplant Program (1992-2008) and am familiar with numerous noted professionals in my field. Dr. Giovanni Ambrosino is one of these extraordinary professionals. I have known him since 1985 and, starting in late that year, he became my go to surgeon to both assist me with my own cases and for me to assist in cases he did during his training. He struck me very early as an extremely talented surgeon, whom I trusted in any circumstance. In addition, he did crucial research in hepatocyte (the intrinsic liver cell) regeneration and pancreatic cell transplantation. Subsequently, he continued to do exceptional, cutting edge research in pancreatic cell transplant and bio-artificial liver. His bio-artificial liver was awarded a patent and was successfully used as a life-saving apparatus. In the clinical field, he became one of the best and most skilled transplant and HPB surgeons, widely respected and a member of the most rarefied group at the top of this field. In his areas of expertise noted above, Dr. Ambrosino has achieved numerous internationally celebrated and major original contributions of great significance to the medical field, particularly liver transplantation and treatment. Specifically, he has performed domino transplants (these can be, for instance, cases in in which a recipient’s own liver is not necessarily damaged, but causing damage to its owner’s body due to metabolic problems and is replaced via transplant, but then the removed organ can be donated to another patient on the list who will not be affected metabolically). He also pioneered robotic surgical techniques that have advanced this newer field of surgery.
