all other medical floors. do FL-2’s, obtain passr (Level I/Il), identify readmission risks, complete

psychosocial assessments, case management initial assessments, transition notes, discharges,

collaborating with multidisciplinary care team, identifying discharge needs/safe discharges/. Accessing

internal/ external resources to meet patient's needs, assists with guardianship paperwork, advanced

directives, homeless shelters/ placement/ placing patient’s in SNFs/ ALFs/ working with group homes,

other facilities, behavioral health, placing patients in geri psych/ adolescent psych/ identifying patient

/family crisis intervention & providing emotional support and effective plans for resolution, initiate end

of life discussion with patients/ families/ physicians/ assisting with setting up hospice/ SECU/ home / and

home health referrals. Assisting with resources, indigent drug programs, identifying ethical issues,
