Attending Paediatric Physician/Specialist Paediatrician.

SMC Hospital is located in the heart of Riyadh the capital of Saudi Arabia, has become one of the modern landmarks in Saudi Arabia since its inception in 1999. The SMC hospital consisting of over 450 inpatient beds and over 150 outpatient clinics is served by a dedicated and highly qualified international team of over 300 full time physicians and 1,600 paramedical and support staff. The SMC hospital provides consistently successful treatments to all types of patients with the international standards of comprehensive health care. Over the years, SMC hospital have built a sound foundation to remain in the forefront of the Health Care Sectors in the Middle-East.

My current responsibilities are: management and treating all types of paediatric patients daily attending in Paediatric Emergency Department (PED) and In-patients Ward, providing advice to all In-patients referrals from other departments of SMC hospital, providing independent clinical care and ensuring that optimum care is provided by following clinical governance protocols, managing critically ill-patients. Assessing & management of children in PED and in Short Stay, taking all decisions by myself, admitting the patients as per need, replying consultations from other departments regarding the patients of Pediatric age group, giving the necessary management for conscious sedation of children for different investigations, management of Coronavirus patient in emergency, appending newborns who born in PED, dividing the patient to different sub-specialties, writing order for admitted patients for 24 hours until General Pediatric Consultant comes and occasionally I am responsible for all the admitted Pediatric patients of the whole hospital, reviewing patients seen by trainee doctors for decision making. Admitting and treating patients as part of hospital’s on-call services, diagnosing issues and implementing treatment plans to resolve problems or help manage symptoms. Ordering and executing various tests, analyzing & interpreting test results, diagnostic images to provide information on patient symptoms. Also, appropriately, communicating with other consultants and colleagues in the nearest hospitals in order to manage patients. Supervising and managing daily activities of clinical team consisting of junior resident physicians, nurses and support staff. Providing education/clinical supervision and training of junior doctors & medical students by promoting a teaching environment in our hospital. Responsibilities were teamed with physicians, specialists and health care providers to provide well-rounds care for complex cases, management and treating all paediatric patients daily attending our PED and In-patients Ward, providing advice to all In-patients referrals from other departments of SMC hospital, administered required vaccinations and advocated for adherence to vaccination schedule for the best disease prevention. Provided comprehensive medical care for children, including yearly physicals, sports physicals, immunizations and growth monitoring. Admitted and treated patients as part of hospital’s on-call services, diagnosed issues and implemented treatment plans to resolve problems or help manage symptoms. Ordered and executed various tests, analyses and diagnostic images to provide information on patient symptoms. Supervised and managed daily activities of clinical team consisting of junior resident physicians, nurses and support staff. The KFMC is the largest tertiary care facility in the Middle East that incorporates a General Hospital (600-bed) including Busy Cardiac Surgery & Haemato-Oncology Centre, Maternity Hospital (400-bed), Children's Hospital (300-bed) and a Rehabilitation Hospital (200-bed) situated in the Centre of capital Riyadh in Saudi Arabia. This a JCI, CAP and SCBAHI accredited hospital, annually, KFMC provides treatment facilities for more than 100,000 In-Patients and 800,000 Out-Patients. And Its Paediatric Emergency Services is a state-of-art well equipped department with modern facilities.

This post improved my training in paediatric emergency medicine exposing me to new areas of acute and other emergencies, this is the largest emergency facility in Saudi Arabia, in which, I acquired skills of rapid assessment/ prioritization of large numbers of patients especially in busy admission periods. My responsibilities were: taking care of all paediatric patients in Triage and in PED under consultant’s supervision, prioritize the patient from the triage to the PED in Children Hospital of KFMC, providing direct clinical care to the patients attending in PED and ensuring that optimum care is provided by following clinical governance protocols, managing critically ill-patients and also reviewing patients seen by trainee doctors for decision making, providing education/clinical supervision & training of junior doctors by promoting a teaching environment in our PED, overseeing the management of patients in all areas of PED, involving hospital’s serious incidence investigations and patient's complaints review, participating regular departmental, Clinical Governance, mortality review. I was a member of team following service improvement projects in our PED like emergency X-ray review patient safety net quality improvement project. The Dhaka Shishu Hospital is first time set up a largest dedicated children hospital in Bangladesh. This post was my first introduction to paediatric medicine decided my choice of career. As a non-paid physician in Pediatrics, there I gained extensive clinical work experience in general and emergency paediatric patient management as well worked in NICU & PICU.

In seven months period I was attached in the Children Hospital of KFMC and received extensive clinical training in General Pediatrics as well as Adolescent Medicine.

Practical Skills and Procedures

I can perform the follwing procedures independently-

For neonate:

1-Initial steps of resuscitation of newborn -provide warmth, clear the airway, dry and stimulate the newborn

2-ventilation

3- chest compression

4-administration of epinephrine and or volume expansor

5-use of pulse oxymetry

6- temperature control

7- assessment of oxygen need and administration of oxygen,

8 arterial blood gases analysis and

9- endotracheal tube placement.

For infant and children:

•Resuscitation of sick patients

•ventilation

•Ultrasound Procedural skills including ultrasound guided IV placement, intraosseous placement, intubation, procedural sedation, laceration repair techniques, splinting and casting.

•Child CPR-Synchronized cardio version and arterial insertion.
