Rick Carlstedt’s Involvement in Greg Carlstedt’s treatment:

*/**/**** – Began Suboxone treatment from Dr Vedak. No support of involvement from Richard Carlstedt.

2/13/2015 – No contact on my Birthday. No call, no text, no explanation as to why.

3/01/2015 – Still no contact, no involvement, and no support from Richard Carlstedt.

3/31/2015 - Still no contact, no involvement, and no support from Richard Carlstedt.

4/11/2015 – Dr Vedak’s Treatment Plan is not followed when Jan Carlstedt advocated strongly to involve Nancy Keucke, a social worker. GMC denied numerous requests to see Bud and Nancy Keucke. This was due to Bud and Nancy not having the appropriate certifications to treat Greg Carlstedt. Please see text messages attached.

Deviation from Vedak’s treatment plan led to an onset of unmanageable anxiety. An ER visit was recommended by Nancy Keucke. Colleen McKernan later reported that “no suboxone patient should ever be sent to the Emergency Room without an advocate.” Colleen reported that Greg Carlstedt would likely not have been put on a medical hold if proper suboxone treatment procedures were in place by Nancy Keucke. Colleen is a therapist who is certified in addiction, suboxone treatment, and EMDR. Nancy Keucke is a social worker that has no addiction or suboxone certifications.

“Nancy/Social Worker/Friend of Family: released trauma with EMDR, caused intense abandonment to be activated by going out of town on vacation for two weeks with no therapeutic coverage, she had no addiction background: misrepresented Greg’s current state to his parents and Father Jim, Greg described this as “devastating”, reports he has fired her Missing sister’s wedding, missing friend’s wedding in Wisconsin.”

5/01/2015 – No contact, support, or involvement from Richard Carlstedt.

06/01/2015 – Limited support, little to no contact from Richard Carlstedt.

6/17/2015 – A mediated phone converstion between Jan, Richard, and greg Carlstedt, and nancy Keucke. Plans for Kristen Carlstedt’s wedding are discussed, as well as Greg’s involvement in the wedding. Richard and Jan Carlstedt agreed to follow up with the following:

Will send GMC invitation and instructions for tuxedo, where to stay, place in wedding.

Will help Kristen Carlstedt contact GMC with additional wedding information.

Travel plans surrounding the wedding are not discussed.

Nancy Keucke and Greg Carlstedt waited two weeks for Jan, Richard, and Kristen Carlstedt to follow through on the plans laid out during the phone call. Richard and Jan Carlstedt did not follow through with the agreed plans. An explanation as to why they did not follow through with the plans they agreed to has not been provided.

07/01/2015 – Greg Carlstedt sustained shoulder injury while weight training.

- Nancy Keucke referred Greg Carlstedt to the Intensive Outpatient Program at Centennial Peaks.

7/10/2015 – Intake meeting at Centennial Peaks occured. Denied placement in Substance Abuse program due to patient being sober for months. Placed in Mental Health program. This was a bad fit for Greg Carlstedt’s needs at the time.

Rick Carlstedt became involved in Greg Carlstedt’s recovery and rehabilitation some time in July of 2015. Greg began treatment in Early February of 2015. Rick Carlstedt will not provide an explanation as to why he did not become involved or offer support for the first 6 months of Greg Carlstedt’s treatment.
