Job Summary:
Responsible for delivering direct patient care utilizing the process, to include assessment, implementation and evaluation of care required of the assigned patients. Promotes and restores patient’s health by establishment of a plan of care and educating patients and their families about their conditions. Collaborates with physicians and multidisciplinary team members, providing physical and psychological support to patients and families. DUTIES AND RESPONSIBILITIES:
1. Works with variety of patients, or specializing in one of numerous possible areas based on experience and competency.
2. Provides direct patient care as an effective team member and promotes the highest standards of clinical nursing care established on evidence based practice. 3. Monitor patient progress and identifies any change in status, acting on those changes to insure patient comfort and safety to include interpretation and evaluation of diagnostic test to identify and assess patient’s condition.
4. Perform routine nursing care to assigned patient, while utilizing age specific and standard precaution practices at all times.
5. Maintain accurate clinical documentation and patient information systems ensuring the data and complete utilizing the documentation system that is used by the service scope. 6. Maintain records for patient cares accordance to established policy, procedure and protocols. 7. Recognize and uphold the rights of all patients and family members accessing the health service and advocate on their behalf. This includes maintaining cultural sensitivity. 8. In collaboration with physicians and all healthcare professionals caring for patients. Always give a complete and thorough report on patient status at end of the shift/assignment using effective interpersonal communication.
9. Modify patient treatment plans as indicated by patient’s responses and conditions in collaboration with the interdisciplinary health care team.
10. Practice patient teaching/training for patients and families. 11. Follow hospital policy when disposing of biohazardous materials, labelling ad handling of all materials. 12.
