● As a Hemodialysis (HD) RN at the U of MN during the onset of SARS-Covid-19 pandemic, duties ranged from setting up and knowing critical concerns with the Fresenius 2008T HD machine, accessing and caring for Arteriovenous Fistulas and Grafts (AVF & AVG) with different gauge needles as ordered plus applying pressure when needles removed for achieving hemostasis. Also accessing and caring for Central Venous Catheters for HD use, including flushing, saline, tPA, citrate or heparin locking catheters, and weekly or as needed sterile dressing changes. Important nursing criteria for performing HD included watching for instability of hemodynamics with each patient. While performing treatment in the HD unit, there was a maximum of 2 patients per RN and only 1 patient per RN while in the ICU. At the U of MN, the patients that were seen for HD were critical and fragile patients, usually with numerous comorbidities.

● Other duties included performing HD treatments in the Intensive Care Unit or in other units of the hospital. While working with some patients, other RN duties were necessary, such as titrating insulin drip doses per algorithms, assisting with NG/NJ tube feedings or administering medications via NG/NJ tubes, managing medications, checking blood glucose, drawing labs, being familiar with HD medications, among other RN duties. Also training and using Baxter SpectrumIQ IV pumps, insulin drip syringe pumps, bolus or pump tube feeding, among other nursing duties relating to personal care of patients.

● Training on ECG was important as patients receiving HD required continuous ECG monitoring during HD treatment as to watch for heart rhythm changes. Independent training from AACN material for ECG class preparation was necessary, then a 1-week long class for ECG training was required pertaining to dysrhythmia monitoring for HD RNs.

● SARS-Covid-19 training for donning and doffing PPE was a requirement during the onset of this pandemic.

● As a HD RN at the U of MN I was scheduled for three 10 hour shifts per week, however overtime was needed consistently, also working every other weekend was necessary, plus on call overnights 3-4 times a month.

● PD RN/Case Manager for up to 34 peritoneal dialysis patients at one time while at DaVita, which includes a dynamic of balancing patient’s scheduling, treatments, trainings on how to perform PD at home on the Baxter Cycler, and also administering intraperitoneal antibiotics, intramuscular vaccinations and other medications for anemia including subcutaneous injections, or intravenous push iron sucrose. From start to finish looking at end stage renal disease patient’s experience starting PD would look like this for my responsibilities: Scheduling a time with patients who need to start dialysis per MD to talk to them about all dialysis modalities. Next, if they decide they would like to start PD as their modality I would schedule a time for them to meet with Vascular Access MD and RN at a hospital of the patient’s choice. Once the patient has scheduled a surgery for placement, the vascular access RN and surgeon will instruct them on how to proceed with having a PD abdominal catheter placed. After the patient has the PD Catheter surgery, I will schedule a sterile dressing change with the patient approximately 7 days after, no sooner. When I have the patient in for the 7 day post operative dressing change, I will schedule training with the patient for a minimum of 8 days and start training 7 days post operative dressing change. The patient must then come into the clinic with me to start learning how to perform PD therapy at their home. Again, training lasts for at least 8 days, four hours per day of training. It is my job to train them how to perform therapy at home, but the importance of learning the science behind PD and how the dialysis works for them. I also train the patient how to perform daily exit site care, aseptic technique used with PD, how their membrane works, how to perform vital sign collection and home record keeping, how to perform collection of samples for a test to ensure they are adequately dialyzed, how to decide what dextrose solutions they will be using during their daily treatments to ensure good fluid balance, how to take certain medications, and how to maintain the best health they are able to. After they are finished PD training and are sent home to perform their treatments at home, then I see them twice a month- once so I can draw monthly labs via venipuncture and they can see the MD to discuss lab values and home therapy.

● I was a float PD RN for one year with DaVita, meaning I went to multiple clinics around the Metro Minneapolis area, all the way from Glencoe, Apple Valley up to Maple Grove and Coon Rapids to fill in if anyone needed help. After I was the float RN, I settled into a permanent position at Saint Louis Park, MN DaVita clinic. But around the same time, approached about a position as the first PD RN Maple Grove DaVita PD clinic while assisting in the opening of the Home Modality, and said yes immediately. I went through the Centers for Medicare and Medicaid Services audit and passed with not one corrective action noted during my audit with the State of Minnesota.
