Quality Program Lead, Arizona Oncology, Phoenix, AZ

Management of Value Based Care (VBC) programs including Oncology Care Model (OCM), Merit Based Incentive Payment System (MIPS) and commercial insurance contracts. Quality Metric alignment, to include VBC programs and Quality Oncology Practice Initiative (QOPI). Collaboration with commercial VBC programs increase quality of patient care. Analyzes Data, identifies process improvements, and develops workflows to implement process improvements to improve patient care. Designs procedures and performs audits to increase data completeness within the EMR. Presentation and submission of quality metrics to meet timelines of VBC participation guidelines. Shares improvement activities, business details to include process improvements, contract status updates to physician and executive leadership. Management of nurse navigators, social workers, value-based care billing staff statewide, using interactive electronic platforms.

Nurse Navigator

Implementation of the Nurse Navigation Program, Inspired by Center of Medicare Services, Oncology Care Model (OCM) in the community oncology setting. Identification, assessment and implementation interventions to overcome healthcare barriers throughout the healthcare continuum for oncology patients. Telephonic management of oncology patients providing therapeutic communication and management of treatment side effects to included adding community resources the plan of care. Implementation and education and care team conferences to increase collaboration and encourage a multidisciplinary approach and increasing patient quality of care. Onboarding and development of staff new positions to support the metrics of value- based payment models. Training, development and mentoring of nurse navigators for Arizona Oncology. Education and collaboration with staff to improve processes and procedures while meeting requirements of various value-based payment models.

Participates in US Oncology Network Clinical Advisory Council, providing leadership in development of standards, implementation of patient-centered coordination of care throughout the healthcare continuum for the Nurse Navigators. Monitoring value-based metrics and identify areas of process improvement. Develop standards to increase the quality of patient care. Presentation of innovative processes to the Clinical Advisory Council and national oncology organizations which support increased quality patient care.

Provides professional nursing care, to the oncology patient in the outpatient infusion center to included telephonic patient interaction. Monitored and managed treatment side effects related to oncology treatments to patients. Maintains adherence to national standards, identified by the pharmaceutical supplier, OSHA, FDA and ONS standards. Advanced knowledge of utilization of intravenous devices.
