Telephonic management of oncology patients providing therapeutic communication and management of treatment side effects to included adding community resources the plan of care. Implementation and education and care team conferences to increase collaboration and encourage a multidisciplinary approach and increasing patient quality of care. Onboarding and development of staff new positions to support the metrics of value- based payment models. Training, development and mentoring of nurse navigators for Arizona Oncology. Education and collaboration with staff to improve processes and procedures while meeting requirements of various value-based payment models.
Participates in US Oncology Network Clinical Advisory Council, providing leadership in development of standards, implementation of patient-centered coordination of care throughout the healthcare continuum for the Nurse Navigators. Monitoring value-based metrics and identify areas of process improvement. Develop standards to increase the quality of patient care. Presentation of innovative processes to the Clinical Advisory Council and national oncology organizations which support increased quality patient care.
