Personal Statement

I maintain a very high level of standard of patient care and this can be verified from my referees. I specialise in anaesthesia and intensive care for complicated and demanding patients. I am a highly motivated, committed and experienced Anaesthetist and Intensivist. I have high level of practical skills gained over many years. I have been heavily involved in management and teaching throughout my career, excelled in my clinical experience and am highly regarded by my surgical, anaesthetic and intensive care colleagues. I am a good team player with leadership qualities who communicates well to all his colleagues. I am a Speciality Editor for a British Journal and have written several publications in Anaesthesia and Intensive care. In addition, I have lectured at International Anaesthetic and Intensive Care meetings.

I have been living in Abu Dhabi with my wife since June 2015. During this time, I have been employed as a substantive Consultant and Head Of Department in Anaesthesia and Intensive Care at Mediclinic Hospital. Then we moved to Dubai in 2019 where I was working in Medcare Women and Children Hospital as Consultant in Anaesthesia and Intensive care. This demonstrates my ability to both transition to a new institution and adapt to and flourish in a new and very different cultural landscape. Summary of Previous Employment

ADVANCED TRAINING (HIGHER PROFESSIONAL TRAINING / CCST):

My Higher Professional Training (HPT) / CCST has been in the post of Senior Registrar in Anaesthetics within the Hammersmith Hospital Training Scheme. This is a major tertiary centre in London.

The First thirteen months of my HPT were spent at the Hammersmith Hospital, which is the site for the Royal Postgraduate Medical School of the University of London. Major surgical specialities are therefore represented, and I gained considerable experience in anaesthesia for Cardiac, paediatric and hepato-biliary surgery. I became involved in some research projects and I learned how to set up research protocols; this provided useful training in research methodology. Also, I was given the opportunity to deliver lectures at large meetings.

The next twelve months of my HPT / CCST were spent at the Lister Hospital (Stevenage). This allowed me to master all aspects of anaesthesia practised in a busy district general hospital. As the sole senior Registrar, this post gave me a chance to organize research, teaching more junior staff and gain management experience. This was followed by rotation at the Marsden Hospital which is a major Oncology hospital in the UK. Here I learnt to deal with major cancer surgery, difficult airway problems and chronic pain management.

BASIC SPECIALIST TRAINING

The major part of my general professional training in anaesthetics was obtained in district general hospitals. My initial training was undertaken at the East Birmingham Hospital where I gained experience in the basis of anaesthesia for elective and emergency surgical procedures. In addition, I was introduced to the specialities of intensive care, thoracic anaesthesia and the problems associated with obstetric analgesia and anaesthesia.

My first Registrar post was at the Edgware General Hospital (Middlesex) which is a busy district general hospital, affording a special interest in thoracic surgery. I continued to gain invaluable experience in most anaesthetic specialities at Glan Clwyd and West Middlesex Hospitals which were both extremely busy district general hospitals. St. Mary’s hospital was my first experience of a teaching hospital. I obtained clinicaexperience in anaesthesia for a variety of surgical specialities (e.g. renal transplantation, urology, vascular, and major ENT surgery).

Relevant Experience

Throughout my career, I have sourced and secured positions both inside and outside of medicine that have offered me the wide range of experience and exposure required to be an effective and capable consultant in a busy hospital environment. I continually update my knowledge through a combination of self-directed learning, external courses and meetings, reflective practice and the pursuit of formal qualifications such as the MBA.

In all my consultant posts I have had regular exposure to flexible anaesthetic lists which have included general surgery, emergency, bariatric, paediatrics, neonates, vascular, ophthalmic, renal transplant, plastics, urology, elective orthopaedic and major trauma cases. I have expertise in advanced airway management, anaesthesia and sedation (including total intravenous anaesthesia), regional anaesthesia and acute pain management.

Summary of Clinical Experience

DAY CARE SURGERY

I have gained considerable experience in anaesthetizing for a wide variety of surgical procedures amenable to day care e.g. termination of pregnancy, herniorrhaphy, varicose vein avulsion, circumcision, insertion of Grommets, carpal tunnel decompression, cystoscopy and Laparoscopic cholecystectomy.

I fully understand the advantages and disadvantages of day care surgery and the importance of patient selection. I have tried a variety of anaesthetic techniques which allow rapid recovery and minimal morbidity.

GENERAL AND VASCULAR SURGERY

I am experienced in the provision of anaesthesia for most general surgical operation including major surgery such as Oesophagectomy, Whipples procedure. I have also gained considerable experience in bariatric surgery in the UK, Oman and UAE. I have provided anaesthesia for elective vascular surgery such as carotid endarterectomy, aorto-femoral and axillofemoral bypass grafts, and have also been involved in the management of patients with ruptured aortic aneurysms, both in theatre during surgery and postoperatively in intensive care. I have also anaesthetised many patients undergoing endovascular aneurysm repairs (EVARs).

PAEDIATRIC ANAESTHESIA

I have had substantial experience in paediatric anaesthesia during both General and Higher Professional Training. In addition to the anaesthetic management of infants and children for otolaryngology, ophthalmic surgery, orthopaedic surgery, thoracic surgery as well as General surgery which included Whipples procedure and hepatectomy. I have experience of anaesthesia in neonates for a wide range of surgical procedures including laparotomy for intestinal obstruction, hernia repair, and gastroschisis.

In my present hospital, I have anaesthetised neonates undergoing pyloric stenosis. This hospital is becoming a major paediatric surgical centre. I have managed children with life-threatening emergencies such as Acute Epiglottitis and Meningococcal Septicaemia.

OBSTETRIC ANAESTHESIA

I have gained extensive experience in obstetric anaesthesia during all my anaesthetic posts. I have received thorough instruction in several methods of pain management during labour and delivery including inhalational analgesia, epidural and spinal anaesthesia, and systemic and epidural opiate administration. I am aware of the problems associated with emergency general anaesthesia in the obstetric patient and the value of skilled assistance for the anaesthetist in such situations.

I have been involved with my obstetric colleagues in the management of the pre-eclamptic and eclamptic patients both in the labour ward and in the Intensive Care Unit. I have had to manage on several occasions major haemorrhage in the obstetric patients. I have extensive training in Intensive Care which has included Obstetric patients. Recently I gave a talk on Maternal Sepsis at an Obstetrics and Gynaecology Conference in Abu Dhabi.

GYNAECOLOGY

I have experienced in anaesthesia for all major and minor gynaecological surgery including dilation and curettage, hysteroscopy, laparoscopy, ruptured ectopic pregnancy, tubal surgery, simple and Wertheim’s hysterectomy and radical vulvectomy.

ORTHOPAEDIC AND TRAUMA SURGERY

My experience includes anaesthesia for major orthopaedic surgery such as knee and hip replacements, shoulder joint repair, and laminectomy involving cervical and lumbar vertebrae. I have also gained experience of anaesthesia for trauma surgery, both minor such as the manipulation of fractures and major such as multiple injury following road traffic accidents. I have been involved with the Enhanced Recovery Programme, which has seen patients being discharged after three to four days following Hip and Knee Surgery.

EAR NOSE AND THROAT SURGERY

My experience in this field includes the management of patients undergoing tonsillectomy, middle ear surgery, laryngoscopy, tracheotomy, laryngectomy, and radical neck dissection. I have worked with surgeons using a laser for laryngeal surgery and I am aware of the precautions which must be taken to protect both the staff and patients. I have also anaesthetized for post-tonsillectomy bleeding and am familiar with the dangers involved. I am able to perform percutaneous tracheostomies on the Intensive Care Unit.

UROLOGY AND TRANSPLANT SURGERY

I have provided anaesthesia for cystoscopy, transurethral surgery such as prostatectomy and resection of bladder tumours, and for renal surgery including nephrectomy and pyelolithotomy. I have anaesthetized patients for cystectomy using epidural blockade both during surgery and postoperatively for analgesia. I am familiar with the problems associated with anaesthesia for patients with renal failure and am experienced in the anaesthetic management for renal transplantation.

PLASTIC SURGERY

Lister Hospital is the regional centre for plastic surgery. I have provided anaesthesia for many plastic operations including corrections of congenital malformations such as cleft palate and microvascular free flap repairs. Patients with burns up to 15% were treated locally. I gained wide experience in the anaesthetic management of these patients undergoing excision burns and skin grafting, and repeated dressings.

DENTAL AND MAXILLO-FACIAL SURGERY

I have experience of anaesthesia for dental extractions, reductions and fixation of fractures of maxilla and mandible, and major resection of head and neck tumours requiring free flap repairs at the Royal Marsden Hospital. I am comfortable using the fibre optic laryngoscope.

OPTHALMOLOGY

My Registrar post at St. Mary’s Hospital was linked to the Western Ophthalmic Hospital where I gained extensive anaesthetic experience for operations such as cataract extraction, retinal detachment, dacrystorhinostomy, squint surgery and corneal grafts. I am familiar with the management of patients requiring surgery for perforating eye injuries and understand the anaesthetic implications of such procedures.

HEPATOBILIARY SURGERY

Whilst at the Hammersmith Hospital, I have anaesthetized patients undergoing major liver and pancreatic resections, and I have also managed such patients postoperatively in the Intensive Care Unit. I have also managed such patients postoperatively in the Intensive Care Unit. On several occasions, I have assumed clinical responsibility of such patients supervising more junior members of the department. At the King Fahad hospital, I have been involved in the anaesthetic and intensive care management of Liver transplant patients.

CARDIOTHORACIC ANAESTHESIA

I have gained considerable experience in cardiac and thoracic anaesthesia at the majority of hospitals at which I have worked. Hammersmith Hospital is the regional centre for cardiothoracic surgery. Over 500 cardiopulmonary bypass procedures are performed annually. These include coronary artery bypass grafting, valve replacement and reconstructive surgery for congenital heart disease. I have been closely involved in the anaesthetic management of adults undergoing valve replacement and coronary artery bypass grafting, and on numerous occasions as the senior member of the anaesthetic team. I am conversant with the principles of cardiopulmonary bypass and appreciate the problems involved in anaesthetizing patients with severe myocardial and valvular disease.

My experience of paediatric cases has generally been under close supervision. I have experienced in anaesthesia for diagnostic procedures and paediatric cardiac catheterisation. I am familiar with the postoperative management of cardiac surgical patients in intensive care and have managed patients who have required emergency re-sternotomy for bleeding following cardiopulmonary bypass. The major part of my experience in thoracic anaesthesia has been gained at Edgware General Hospital, St Mary’s Hospital, and King Fahad National Guard Hospital and Pakistan. I have become proficient in a variety of thoracic anaesthesia techniques including one-lung anaesthesia for lobectomy, pneumonectomy and resection of mediastinal tumours. I have also anaesthetised for and performed rigid bronchoscopy in adults.

NEUROSURGERY

In all my posts, I have gained experience in the management of severe head injuries and have also been involved in the transfer of such patients to neurosurgical centres. I am familiar and experienced in the anaesthetic problems involved for craniotomies for tumours and aneurysms.

REGIONAL ANAESTHESIA

I have frequently used regional anaesthetic techniques intra operatively either as the sole technique or in combination with sedation/general anaesthesia and also to provide postoperative pain relief. Examples of blocks performed include spinal, epidural and caudal blocks, superficial and deep cervical plexus blocks, intercostal and brachial plexus blocks, intravenous regional anaesthesia, sciatic and femoral nerve blocks, and ankle blocks.

CLINICAL EXPERIENCE IN INTENSIVE CARE

At both Registrar and Senior Registrar, I have had clinical responsibility for patients of all ages in a variety of intensive therapy units. I have received instruction and supervision in all aspects of airway management and ventilatory support, and invasive haemodynamic monitoring and support. At Hammersmith Hospital, in addition to the usual range of problems encountered in a general intensive care unit, patients were frequently admitted from the specialised units based there including renal, hepatic, cardiac and paediatric oncology. I have a special interest in managing septic patients. I am proficient in all technical procedures including percutaneous tracheostomy.

I have been the Clinical Director of Intensive Care at Hemel Hempstead for a number of years and I have been invited to lecture on International meetings. I was in charge of Intensive Care at both Mediclinic and Medcare Hospitals in the UAE. I have recently looked after both adult and paediatric patieints in the Intensive Care Unit. I have several publications on Intensive Care topics and also, I have lectured at International meetings.

Recently, I have been involved with the Intensive Care management of Covid19 patients in Dubai.

CLINICAL EXPERIENCE IN CHRONIC PAIN MANAGEMENT

At both Hammersmith and Lister hospitals, a Senior Registrar forms part of the chronic pain management team. At Lister I participated in an extremely busy twice-weekly pain clinic for twelve months whereas at Hammersmith there is a three-month rotation with this aspect of the training scheme. I have also worked at the Royal Marsden Hospital in London where I learnt to manage Cancer pain. During this period, I was responsible for the assessment of both inpatients and outpatients with chronic pain management problems. I received training in a range of therapeutic interventions including coeliac plexus blocks, paravertebral blocks, chemical sympathectomies, stellate ganglion blocks, guanethidine sympathetic blockade, transcutaneous nerve stimulation, cryotherapy of intercostal nerves, and cervical epidural blockade as well as acupuncture.
