RESPONSIBILITIES OF LICENSED PHYSICAL THERAPIST
•Administer physical therapy treatment to clinic/home care/skilled nursing patients/residents within PT’s plan of care to
achieve maximum outcomes for functional independence, decrease care giver dependency and return to PLOF
• Monitor and guide/correct patient performance with exercises and activities throughout treatment session
•Administer modalities to meet short term/long term goals according to POC
•Monitor/record/follow up patient response to modalities pre-treatment, during treatment and post treatment
•Follow safety precautions during patient treatment (such as use of gait belts, hip replacement precautions)
•Record exercises/activities; progress patient; keep PT informed of progress, record progress per company standards
•Keep PT and all team members informed of patient status including recommendations for modifying patient care based on patient progress/response to treatment; participate in weekly staff meeting/case conferences
•Communicate with doctors or other medical personal as needed
•Communicate with patient family members as appropriate and within HIPAA compliance
•Complete all documentation accurately and on a timely basis per APTA / company standards
•Administrative duties/activities as required, including scheduling, insurance verification, supply ordering, etc
•Maintain monitor HIPAA compliance within facility/staff members
