OBJECTIVE:
As an experienced RN BSN, with prior Home Health and Case Management experience, I am seeking employment in Home Health Care and Community Health Care.
QUALIFICATIONS:
I am an experienced RN with more than 20+ years in acute care nursing. My experience includes working in critical care both at Providence and Alaska Regional Hospitals. For the last 10 years I worked at Providence in Home Services. I worked in Home health and then hospice caring for home bound patients with a variety of illnesses or injuries and some at End of Life support. My duties included; medication management, safety measures, how to operate DME’s as well as obtaining orders for equipment needed and ensuring timely delivery. I communicated with other team members to meet all of the patient
needs.
In the fall of 2007 I completed my BSN degree at Kaplan Online School of Nursing, graduating Summa Cum Laude.
OTHER: From 1997 to 2007, with the help of my husband, we owned and operated an assisted living home. This required performing general operations involving client care, staff orientation and development and familiarity with Alaska state licensing regulations. During this time I developed and taught patient care procedures and techniques to the PCA and CNA staff during their orientation and periodically throughout their employment.

EMPLOYMENT:
May 2019 to Feb 2020 Providence Hospice:
Worked as a Case Manager caring for patients on Hospice care with a prognosis of 6 months or less to
live. The approach to Hospice care is a team
approach, including; MDs, MSWs, Case Managers,
Hospice Aides and Chaplains. As a Case Manager I
provided skilled nursing for 10 to 15 patients, which
included assessment of patient needs, providing
DME’s, and training on usage. I provided instruction
and guidance involving using the comfort medication
ordered and provided by the Hospice MDs.
I maintained clear communication between myself and other team members
July 2009 to Dec 2018
Providence Home Health Care:
Worked as a rotating on call staff nurse, providing coverage on the weekends and during the week for case managers overflow patient visits and taking call for the agency after hours, doing PRN visits with adult home care patients as needed.
Home care consisted of evaluation and assessment of patient and environment and providing in home patient/ caregiver education, medication
management, education, lab draws, and wound care for a variety of adult patients.

Worked as a staff nurse with pediatric patients in their homes, many with severe disabilities and who were on home ventilators. Responsibilities included parent/caregiver education and relieving parents/ caregivers for 4-8 hour shifts. Duties involved
thorough assessment, managing ventilators, pulmonary hygiene, suctioning, positioning, bathing and tube feedings.
Also worked as an agency nurse in the state prisons setting up and administering medications, doing assessments and assisting PAs in the clinics.

Sept.1997-Mar. 2004 Administrator and owner of a small assisted living home. Provided total care for four clients with various disabilities until their death.

Alaska Regional Hospital/Medivac Team
Flight nurse. Took call and responded as part of the flight team to locations around Alaska, picking up critically ill patients and accident victims, administering on site medical care and preparing for and transporting by medivac to Alaska Regional or other primary medical facilities around the state.
