The consulting that I did was for the county's juvenile detention centers. My assignment was to find areas that needed to be

improved upon and report back to Spectrum and to the county of my findings. There was an external audit done of the

correctional facilities. One of the areas that were highlighted as "needs improvement" was the nursing departments at both

sites. For many years, there was never a clinical manager in place to ensure the nursing department was running safely,

efficiently and current on the newest best practice standards of care. The auditor identified areas of concern with handling

medications: improper mouth checks for "cheeking", not using any type of patient identification system, not opening the pills

in front of the inmate, and many more. My task was to do research and development of proper handling of medications in a

correctional setting. Spectrum would take the policy and present it to the administration where they take the policy and

implement it. Spectrum agreement with the detention centers was to be hands off during the implementation and evaluation

phase of the policy roll-out.

This was a one-month assignment where I was to cover the correctional facility while the primary doctor was out on vacation.

I provided services at two sister facilities (Spring Mountain Youth Camp at Mt. Charleston) and in town (The Department of

Juvenile Justice). The age range was from 8-21 years old with a combined population of approximately 250 youths. The

duties that were required by the provider to assess diagnose and treat conditions from sleep disturbances and GI upset up to

severe mental disturbances such as homicidal and suicidal ideations with attempts within the facility. The average number of

patients seen within a day ranged between 15 and 20. With collaborative assistance between the psychologist and medical

provider we developed the appropriate plan of care for each of the inmates, parents and parole officers to rehabilitate them to

be a functional member of society. Maintain a structured environment to ensure the youths understood their roles and

boundaries within the facility to help carry this type of behavior once they leave the facility.

Caring Moments principles.

ICU a 4-bed unit that serviced moderately ill patients. Being a rural facility majority of the services was

not available and the patients were sent to the level I trauma centers. Typical patients within this ICU were

status post cardiopulmonary arrest, exacerbation of pulmonary conditions, difficult weaning patients from

surgery. The nurse to patient in ICU is 1:2 or 1:1

Med-Surg. / Med-Tele. The surgical services that BCH provided were knees, feet and hands. BCH nurses

were trained on post-surgical complications of these specific types of ortho surgeries.

Oversee the daily operations of both Intermediate Medical Care Unit (IMC) and Telemetry Services. Approximately 65 FTEs

IMC is a 32-bed step down progressive unit that services the adult population with diagnoses of:

respiratory failure, sepsis, PTCI, post-surgical, et al. Staff development with education in the following

skills: ventilator management, arterial line monitoring/discontinuing. Drip management: Dopamine,

Dobutamine, Amiodarone, Cardizem, Integrilin and additional drips. By leading by example, creating a

positive milieu and being visible to the staff IMC were able to move HCAHPS scores from the 1st quartile

to the 3rd & 4th quartile in one year.

Telemetry Services is a standalone unit that has the capability of monitoring 144 patients. 15 telemetry

employees are competent in using Welch Allyn monitoring system to monitor cardiac & pulse oximetry.

Developed new telemetry policy and procedure after facility sustained multiple sentinel events regarding

telemetry.
