Determine if the medical equipment, which was recommended by the primary care physician, is medically appropriate and if it contributes positive functional outcomes.

Complete a full write-up, report to upper management, and submit report for review and approval by the group and the insurance provider.

Evaluate adults for Occupational Therapy needs status-post hospitalization.

Report directly to regional director regarding case management, recommendations, strategic development of positive outcomes, and durable medical equipment.

Full assessment write-up and transition/supervise service visits with COTA

Provide training to staff, caregivers, and patients for positive therapeutic outcome. Also to prevent decline of functional skills for the patient.

Evaluate and clinically assess geriatric patients with mild to severe orthopedic and neurological disorders in skilled nursing and adult day health settings.

Visited patients in their homes for a full home evaluation to prepare them for the transition to home. This included equipment recommendations, environmental modifications and referral to multiple resources if structural changes needed to be met, home exercise program development, and referral to additional home health services
