Reports to Resident Services Office after clocking in. Receives assignment from licensed nurse, Resident Services Coordinator, or lead caregiver. Learns the plan of care for each assigned resident noting changes and social tasks or observations for that shift.
Follows Operating Guidelines to assure compliance with federal, state, and local regulations and corporate policies.
Encourages residents to be as independent as possible. Assist residents with kindness and patients in the activities of daily living as needed: a. Bathing/showering
Dressing, Grooming, Shaving, Transferring, Toileting, escorting to dining room or activities
Provides resident care to each resident in a manner that demonstrates knowledge of residents’ rights. Demonstrated respect for privacy by knocking on the resident’s door before entering. Maintains resident confidentially. Informs the residents of the purpose (i.e. assist with bath, dress) and allows them the right to refuse.
Provides skin/back care as needed. Observes for any reddened or pressure areas that may develop and reports to licensed nurse.
always Keeps incontinent residents as clean and dry as possible; changes bed linens and clothing as often as necessary. Keeps Resident Services Coordinator informed of frequency.
Helps to set up the dining room for meals. Takes meal orders in the dining room and serves residents in a proper manner. Assists in clearing the dining room at the end of the meal.
Assists in delivering meal trays to residents who will be eating in their apartment.
Obtains routine vital signs and weights as instructed.
Assists residents in preparing for tests, x-rays, therapy, physician visits, activities, religious services, etc.
Assists in transporting residents to activities and/or assists as needed in activity programs.
Assures the safety and well-being of every resident by observation and apartment checks.
Sees that resident requests and emergency calls are answered promptly.
Assists in the admission process by welcoming residents and orienting them to the community. Assists in completing inventory list if requested.
Assists in apartment changes or discharges and deaths by assembling or transferring resident belongings as directed.
Documents all care supplies used by resident for charging.
Assists residents in keeping their apartments orderly. Makes bed daily except for housekeeping weekly changes. Sees that resident’s personal items are stored properly. Takes dirty linens to collection hampers, making sure no glasses, teeth, supplies or equipment are in the linen. Extra linen is not to be stored in the resident’s room.
Participates in infection control by careful hand washing and infection control procedures.
Assists residents with medications as needed when a licensed nurse is not present.
Documents all resident care on specified forms. Reports all unusual incidents and change in the resident’s condition to the assigned licensed nurse. Documents on specified forms.
Reports all hazardous conditions and defective equipment to the licensed nurse immediately.
Shows respect and responsiveness to residents, their families and guests. Solves any resident or family complaint when possible and reports others to the assigned licensed nurse.
Answers telephone as necessary using proper telephone etiquette and writing accurate messages.
Attends community in-service classes and staff meetings on a regular basis.
Works as a Chancellor Place team member to maintain a pleasant, effective work environment for staff and caring home environment for the residents.
When assigned to the night shift, works independent of licensed nurse using Operating Guidelines for when to call 911 or community staff.
Participates on committees as assigned.
Performs other duties as assigned.
