Community Health Worker

Responsible for helping families with small children navigate and access community services, and adopting healthy behaviors.

Manage a case load up to 40 clients who are women with children between the ages of 0-12 mos.

Clerical duties including answering the phones and use of basic computer skills.

Field work include doing home visits setting up pack and plays and doing activities with the children

Provide social support and informal counseling, and advocate for individuals and community health needs.

Providing ongoing follow-up, basic motivational interviewing and goal setting with patients/families.

Conduct intake interviews with patients, including enrolling and/or referring patients into care.

Follow-up with patients via phone calls, home visits and visits to other settings where patients can be found.

Assist patients with completing applications and conducting eligibility determination, enrollment, and follow-up services.

Help patients set personal goals, and attend appointments; Provide referrals for services to community agencies as appropriate.

Help patients connect with transportation resources and give appointment reminders in special circumstances.

Exhibit excellent working relations with patients, visitors and staff.

Work closely with medical provider to help ensure that patients have comprehensive and coordinated care.

Cooperatively with other clinical personnel assigned to the same patient.
