seen was mostly THC primary care physicians associates. Patient
were post-discharge hospital requiring continuing skilled care and
rehabilitation from 20-30days or transferred over to long-term skilled
nursing. The skilled nursing and rehabilitation visits consist of

labs, and medications. Entering all patient information and plan of
care into Next Gen electronic medical records. Keeping track of
hours within THC electronic system. Discuss plan of care with
primary physician Duties in office consist of two days a week
patient range from middle age adult to-adult geriatric seen for such
as cold, flu, allergies, COPD, Diabetes, EKG, and etc. Prescribe
medications, order labs, and specialty consults referral, on call for
patient in skilled nursing facilities.
