2013 – Current East Bay Community Recovery Project – Personal Service Coordinator Used the ACT Model, a no harm reduction program to address the needs of our partners. Also, worked collaboratively with treatment team, Criminal Justice Mental Health (CJMH), and the larger community in the delivery of mental health services, housing, vocational and substance use rehabilitation, to address the needs of the at-risk population we serve.

Provides support/outreach to our partners at Santa Rita County Jail on a bi-weekly basis, as well as attend court hearings.

Manages the CRISIS PHONE LINE and accepts all emergency calls from 4pm to 8am for one week each month.

Instrumental in maintaining interdisciplinary notes, assessments and treatment plans on Clinicians Gateway.

Composed court letters to behavioral health court on behalf of our partners indicating their successes and failures.

Assisted them in shopping, ADL’s, bus and BART passes, housing, DMV, SSI, HAC appointment, General Assistance, Food stamps/EBT Cards etc.

2007– 2012 Napa State Hospital, Psychiatric Social Worker Worked collaboratively with Interdisciplinary team and medical staff in the Forensic Department and Skilled Nursing Unit at Napa State Hospital. The targeted populations included men and women from 19-98 who were Penal Code 1370 – “Incompetent to Stand Trial”, PC 1026 - “Not Guilty By Reason of Insanity” and PC 2972 “Severely Mentally Disordered Prisoners who pose a threat of danger to the community-at-large. A large percentage of the individuals served were diagnosed with a co-occurring disorder and are challenged with both mental health and substance use.

Evaluated social, emotional and educational, needs of the individuals; Performs outreach and case findings to identify potential psychosocial needs;

Taught groups on activities of daily living, coping mechanisms, communication skills, anger management substance use etc.;

Conducted mandatory reporting activities (TARASOFF and notified APS, elder abuse reports);

Prepared IDN’s or referrals to document case activity;

Provided supportive counseling, individual therapy, and crisis intervention;

Interviewed clients, their families and/or significant others to obtain intake information;

Provided discharge planning by analyzing individual needs and meeting them where they are;

Provided educational information on health issues, substance abuse and nutrition and completed a referral to appropriate medical professional;

Maintained confidentiality of HIV patients;

Acted as a resource for other staff by providing training and consulting on psychosocial issues to enhance treatment of clients;

Kept an in depth log of required assessments and reports that were time sensitive;

Facilitated meetings with CONREP and/or Conservators seeking a temporary and/or permanent conservatorship, or discontinuance of placement.

2001-2005 Child Care Supervisor – Alameda County Assessment Center

Contracted through Kairos Unlimited, by Alameda County Social Services Agency to manage and provide safe 24 hour emergency care and placement for children and adolescents 1 day -19 years old; who were removed for allegations of abuse (i.e. emotional, physical or sexual).

Helped purchase and chose furniture; hired and fired staff; provided positive discipline; training; completed background checks and Live Scans; kept inventory; brought grocery, food, clothing and household items; reconciled receipts and expenditures; and managed personnel time records.

Provided brief medical assessment, psychological assessment and made additional referrals to the appropriate personnel as needed.

Provided nurturing safe environment, where the children were provided a new set of clothing; a shower; a warm meal; and talk or play therapy to help reduce the trauma of being removed from family.

Worked collaboratively with Alameda County Children and Family Services staff, Law Enforcement Personnel, and Emergency Response Social Workers to provide the best possible outcome for placement in the least restrictive environment.

1999 -2001 Kairos Unlimited, Inc. – Case Manager Provided case management, group and individual counseling as well as academic counseling 24 hours a day to adolescent teenage female 13-18 years old. These youth were wards of the court and deemed to be at risk.

Organized seminars delivering advice on avoiding drug use and sexually transmitted diseases. Provided workshops featuring techniques for effective resume writing, how to apply for employment, tips for wiser shopping, banking and effective budgeting.

oPositive program resulted in: fewer pregnancies, no discovered cases of HIV, decrease in drug use and sexually transmitted diseases, and a higher number of enrolled college students.

Worked in conjunction with Child Protective Services, Law Enforcement, Juvenile Hall and their parents;

Accompanied the youth to Court Hearings as many had dual standing in the Social Services and Criminal Justice;

Met with various School Districts & Personnel to obtain an (IEP) Individualized Education Plan;

Provided Wrap Around Services for client based upon individual needs ;

Worked with Summer Youth Programs to identify youth eligible for Summer Job Placement and Fundraising events.

Ensured each youth was signed up for independent living skills programs. Also, assisted with college applications, and housing prior to emancipation.
