For patients enrolled in the Emergency Department Disposition Support (EDS) Program, emergency medical services provider will:

Provide services on an as-needed basis until the patient has a follow-up appointment;

Perform a safety assessment of the home and environment;

Perform a medical assessment;

Perform basic vitals including glucose checks, peak flow monitoring, and patient weight

Perform medication reconciliation;

Assist the patient with their home medications as directed by the patient’s PCP;

Perform assessments and troubleshooting of bladder catheters, PICC lines, and Peg tubes;

Assist the patient with transportation to and from their appointment if needed.

Emergency Disposition Support Program (EDS)

Using the Emergency Department (ED) as a primary care source is becoming more common for elderly patients. Not only is this an expensive avenue for care, but too often it is not the place for them to receive the most timely and appropriate care. In many cases, timely appointments with primary care physicians or coordination with other Henry Ford Health System (HFHS) resources would be the optimal choice for these patients.

To provide clinically appropriate alternatives and to align patients with ambulatory resources that allow them to receive immediate and proper care, HFHS has launched the Emergency Department Disposition Support. The initial pilot of the Emergency Disposition Support (EDS) program is at Henry Ford Hospital and will focus on Henry Ford Affordable Care Origination (ACO) and Health Alliance Plan (HAP) Medicare Advantage patients, with the possibility of future expansion to other Emergency Departments. The focus of the EDS program is to provide value-based care to ambulatory patients in the emergency department presenting acute conditions.

The construct of the EDS Program includes an EDS Navigator embedded in the ED. For relatively acute patients who are being considered for the Inpatient Department (IPD) or an Observation (OBS)) stay, this navigator will investigate available outpatient resources at Henry Ford Medical Group (HFMG) clinics and in the community to route these patients, as directed by the ED provider. The navigator and ED provider will then discuss these options with the patient to ensure that the patient receives the right care, at the right time, in the right place.

The navigator will coordinate urgent appointments with the patient’s primary care physician or with a Henry Ford Comprehensive Care Center as appropriate. Transportation resources will be made available and will be coordinated by the navigator. Approximately two-three urgent primary care appointments are expected to be scheduled across the (HFHS) system each day.

The EDS program is a valuable opportunity to ensure that patients receive the right care in the right place at the right time. The result is expected to be greater patient health outcomes and cost savings for all involved.
