Clinical Case Manager – Supportive Housing/PATH Program

Provided case management on a monthly basis with a caseload of 30 clients with various mental illnesses (paranoid/schizophrenia, manic depressant, bipolar, depression, attention deficit disorder, mood disorder, anxiety disorder, post traumatic stress, etc)

Linked clients to mental health programs, welfare, social security, Medicaid, Medicare, Charity Care, long/short term housing, drug/alcohol centers, and rehabilitation programs.

Provide clients with appropriate therapeutic interventions; write treatment plans/progress notes in AWARDS system on daily basis.

Provided clients with medication education, traveled to different locations on a weekly basis to recruit individuals for the program (soup kitchens/salvation army/train stations etc. Met and exceeded monthly enrollment quote of clients.
