Clinical Experience at CT Facilities: High View Health Center, Ridgeview, Westfield, Riverside health and Rehab, Rockville General Hospital, Tracheotomy care, vital signs, general assessments, Foley care
United Community & Family Services
Manage BHS Medication Refill Line-direct to appropriate provider for refills to fill gaps
Phone in medication refill to pharmacy/verification on orders. Assist client with prescription needs/refills.
Interoffice exchange email/scan notes to providers offsite- med issues to be resolved
Triage all med related phone calls (5 off sites including primary site) for 9 providers
Complete Prior Authorizations to State-interoffice fax
Review abnormal Lab Results and report to APRN or MD for review
Manage Patient Assistance Program within CT guidelines for Documentation for meds
Obtain Vital Signs as requested and Glucose monitoring, Injection Protocol based on provider request
Collaborate with Drug Reps, School Nurse, WIC Colleagues, DCF and other medical departments as needed. Personal Messages for Providers/outside agencies, Family Members, etc.
Actively Participate in Meetings & Performance Improvement Efforts- Committee’s served on include-Infection Control(BHS Representative) Reception Refocus Group and Workplace Violence Committee
Respond to Pager for appropriate office Emergency- ex. Waiting room- client ill in office
Document notes appropriately in chart and in electronic medical record(Greenway)
Manages templates of all Physicians to ensure appropriate availability and works with BHS management to develop strategies to meet client demand
Generations Family Health Center
Administration of immunizations and medications
Triage sick phone calls and walk ins on daily basis
Provide patient education based on plan of care
Track Coumadin levels in standardized ranges
Visiting Nurse Association
Working directly under the supervision of the RN and reporting directly to the RN
Reporting any change in patient status to the primary care nurse
Completing all applicable paperwork and sending these forms directly to the main office
Fax applications to different departments in house
