Develops implements and evaluates orientation in conjunction with the

Director of Therapy, HR and Education Team. Provide supervision and

direction for field staff and act as liaison between field and office

staff. Coordinates weekend field visit assignment including: clinical

triage of visits together with Therapy Team and Scheduling, coordination of

weekend visit completion, update of weekend/holiday logs with communication

to Scheduling and other departments as needed, and follow up coordination

together with Therapy Team and Scheduling for primary therapist visit

assignment. Update Clinical Record on an ongoing basis as relates to

communications, orders, re-hospitalizations, conflicts, etc. Evaluate and

address customer concerns, including documentation and resolution of same.

Communicate with marketing personnel re customer issues that arise and may

affect referral source relationships. Evaluate client episode to determine

appropriateness of discharges versus recertification and initiation of

orders as appropriate. Review staff schedules to assure productivity,

communicating with patient care coordinator of staff availability, as well

as providing Director of Therapy with projected productivity for staff

once/week. Obtains physician orders for field staff from physicians, and

resumption of care orders. Participate in staff meeting agenda and in-

services. Distribute monthly staff meeting information to staff not

attending. Meet with assigned staff as needed to receive client updates and

assure that discharged charts are updated by field staff. Perform annual

performance evaluations on field staff. Provide field training in Start of

Care processes and documentation as Therapy Start of Care (SOC) Mentor as

needed. Coordinate PTA/COTA availability for assignment to see weekend

therapy open patients as appropriate. Supervision of assigned field staff.

Monitor patients clinical records concurrently and retrospectively,

utilizing agency thresholds and indicators. Audit clinical records for

completeness on an ongoing basis and works with professional clinicians to

ensure compliance and comprehensive documentation. Work with professional

clinicians in developing the plan for patient frequency of visits that are

clinically appropriate for the patient. Assist agency personal in care

coordination of patient/client services. Serve as a liaison between the

organization, referral sources, patient/clients and agency personnel.
