CLEVELAND STATE UNIVERSITY: Academic degree in Arts and Sciences, majoring in Social Services. Minor in communications, Certification in Gerontology. Obtained degree while employed 30-40 hours per week. Graduated in 1985.

CENTER FOR HUMAN SERVICES: Main focus of Bachelor’s level practicum. Provided outreach to low-income, inner city elderly residents of Carter Manor, a HUD, low-income housing unit. Also supervised Cuyahoga Community College Mental Health students.


CAREFREE CATERING: 1975-1986. Family business. Worked as a caterer as needed.

WELSH NURSING HOME: 1977-1988. Over ten years at this facility in the capacity of dietary, nursing assistant, maintenance, and housekeeping/laundry. Worked in this facility while completing high school diploma and college degree. Also worked part-time at this facility while working at Shangri-La Health Care Center.

SHANGRI-LA HEALTH CARE CENTER: 1987-1990. Director of Social Services for 238 bed, multi-level nursing facility. Responsibilities included Medicare and Medicaid reimbursement, visiting of residents in Social Services capacity, family and group interaction, Alzheimer’s support group, and organizing a support group for the younger residents of the facility. Active in Admissions, Marketing and Census Development.

GLENDORA HEALTH CARE CENTER: 1990-1992. Hired by Administrator of Shangri-La upon purchase of her own nursing facility. Director of Social Services working part-time while employed at Western Reserve Area Agency on Aging. Assisted with development of the marketing and admissions plans.

WESTERN RESERVE AREA AGENCY ON AGING: 1991-2007. Previously employed in the PASSPORT unit providing the administration of home care services to the elderly, low-income residents on Medicaid living in the community. Hired as an assessor for five years, then promoted to Case Management Supervisor. Supervised eleven Nurse and Social Worker Case Managers to maintain consumers’ level of independence to remain home in the community.

HARBINGER HOSPICE: 6-11-01-6/11/03. Two years employment in the capacity of LISW at this agency serving terminally ill patients. Work included psychosocial assessments, bereavement assessments, bereavement support groups, supportive visits to patients in Nursing Facilities and in the community and documentation in accordance with the Joint Commission Accreditation. Worked in this capacity on a part-time, on-call basis.

FAMILY COUNSELING CENTER: 12/22/03-3/31/06. Recently signed contracts to provide supportive and educational counseling to adolescents, couples and adults as LISW counselor. Personally obtained eligibility for the agency to become eligible to bill Medicare for their services. Worked with court-appointed persons regarding anger-management and relationship issues.

FRESENIUS MEDICAL CARE: 7/30/07-Present. Hired as one of the facility Social Workers to provide support and services to renal dialysis patients. Active with

Admissions, financial issues, resources among other tasks. Also covered for SW’s at other sites while working full time in two departments. Achieved the MVP Award 4 months after gaining employment. Assisted in the Peritoneal dialysis unit as interim Social Worker, working as a team to successfully pass inspection for the Department of Health. While working as Social Worker at Fresenius, patient satisfaction increased in every category on the “Patient Satisfaction Survey” for Social Work. Promoted to Lead Social Worker for the Eastern Ohio region. Duties as Lead include Billing Management for dialysis patients, Initiative to deal with patient depression, Supervision of Social Workers, interacting with RVP and Area Manager team.


CENTER FOR HUMAN SERVICES: Supervised Associate Degree Mental health students in an Outreach Office.

SHANGRI-LA HEALTH CARE CENTER: Director of Social Services. Supervised the Admissions Coordinator and other Social Workers.

GLENDORA HEALTH CARE CENTER: Social Worker overseeing the Admissions Coordinator and the Social Services Designee.

WESTERN RESERVE AREA AGENCY ON AGING: Supervised twelve Nurse and Social Work Case Managers in a five county area.
