A Matter of Balance Program is aimed at people who have fallen in the past, have a risk of falling and who may be reducing their activities due to a fear of falling. Through A Matter of Balance Program participants learn to change their environment to reduce fall risk factors, to view falls and the fear of falling as controllable, to set realistic goals to increase activity, and to exercise to increase strength and balance. Partners in Care Foundation has become the only organization in the Western United States to receive new grant funding aimed at reducing the rapidly escalating number of older Americans at risk for falls. The Lay-leader Coach is responsible for leading the program workshop, following the established curriculum in a team teaching environment, interfacing with medical professionals and community representatives to enhance the mission of Partners and the success of the program. Through The Healthier Living Program (Stanford Chronic Disease Self-Management Program) participants develop practical skills for living a healthy life and management their chronic conditions. The Lay-Leader Coach is tasked with facilitating the workshop within the curriculum guidelines. Evaluating existing treatment program, implementing health strategies complementing the treatment team’s goals.
