Objective: Applying for the position of Clinical Pharmacist and Management where I can utilize my skill to promote safe and appropriate optimal patient care.

Summary:

Pharmacist with extensive experience in hospital, retail, and ambulatory Care Clinic.

Discharged patients using face to face to communicate and make interventions.

Conduct medication reconciliation for inpatient and outpatient settings, and evaluate the domains involved in the transition of care process for newly discharged patients.

Participate and several foreign medical and non – medical mission trips.

Establish and maintain cooperative working relationships with Physicians, Nurses, Administrators, and other health care industry personnel.
