Training/teaching of junior resident doctors and other allied health workers that are relevant to the primary the health care setting.

My job duties are further explained below:

Healthcare Services:

Emergency Medical Services: I managed multiple trauma patients, various type of shocks with background hemodynamic compromise.

Endocrine and metabolic emergencies, hypertensive emergencies.

Emergency obstetric services (EMOC), et cetera.

Outpatient care: Areas where I worked include:

Management of communicable and non-communicable disease, Continuity of care for other clinical specialties.

Palliative care across the age groups, Geriatric medicine, Patient education/counseling.

Carried out home visits as required and Pharmacovigilance.

Basic Laboratory Services: This include blood sample collection/ handling, other specimen collection and basic urine analysis,

The use of clinical microscope and interpretation of laboratory results.

2. Research/Project: I participated in the various research project in the hospital that touched different aspect of healthcare deliveries in Federal Medical Center Owo.

The implementation of a project outcome improved patient waiting time at General Out-Patient Clinic (GOPC) and at Emergency Medical Services (EMS) by over 50%.

The patient waiting time was estimated to be 2.5-4 hours by documentation of check-in time on the appointment cards from the central medical record to the time seen by the resident doctors at the outpatient clinic.

The perceived causes of delay in patients flow system within the hospital that resulted in increased waiting time include:

layout of clinic (a central medical record and a payment outlet for the hospital),

patients waiting in the lobby for vitals from a nurse most times to 2 nurses (average time spent on vitals/patient is 5-6mins),

Clinical area holding queue (resident doctor busy with a patient- average of 7-12 mins).

The implementation of the outcome involved:

Locating a payment and medical record unit directly at the outpatient clinic. This reduced the total walking distance and avoiding long queue of patients by 15-25mins.

Continuous rearrangement of patient’s case note at the clinical holding area by crosschecking check-in time before presentation to the resident doctor. This ensured that patient that checked in earlier were promptly attended to.

Manpower was added to the nursing area to decongest patient waiting in the lobby for vitals which reduced waiting time by 20-30mins.

3.Teaching/Training:

Assisting and guiding junior residents daily to review patients before making final decisions on treatment modalities.

Guiding junior residents and other allied worker and those undergoing Continuing Professional Development (CPD) to recognize limitations and prompt referral of difficult cases.

I coordinated morning reviews and auditing of cases managed by residents at the emergency room.

Teaching of core family medicine values including patient- centered care (PCC) and how to manage difficult/complex patients.

I review clinical case write up with family medicine residents and also participated in Journal Review.

OBSERVERSHIP PROGRAM staggered over 12 months at Quality of life Hematology and Oncology Clinic, Johnson City, Tennessee under the supervision of Charles Famoyin MD.

I assisted in electronic medical records, medical filling / transcription and as a medical assistant.

This health center was built by the Federal Government of Nigeria and situated in a remote rural setting which served over 25 other hamlets/small settlement within a radius of 10-25 miles apart).

Job duties:

Provision of basic health care services to all age groups and administration of free health program for the under-fives and pregnant mothers.

Identification and management of malnutrition and failure to thrive in the under-fives and prompt referral of severe cases

Risk identification and management of hypertension in pregnancy (PIH), risk identification and diagnosis of obstructed labor and prompt referral of severe cases.

Screening for HIV in the under-fives and pregnant mothers.

Disease notification and reporting of immunization coverage to the ministry of health.

Community assessment: geared towards proffering solution to health related problems in the community within the context of primary health care setting.

Administrative duties including supervision of nurses, community health workers (CHEW), laboratory technician, pharmacy technicians numbering about 24 under my jurisdiction.

Monthly report of basis health indicators, income/ expenditure and logistics directly to the Permanent secretary Ministry of health Ondo State.

The relevance of the dissertation: This is due to the paucity of information in the area of family support and self-care practices in menopausal women in Africa including Nigeria.

Studies have predominantly investigated self-management interventions relating to specific diseases or long term conditions particularly hypertension, diabetes, asthma, arthritis, but none have addressed areas concerning family support and self-care in menopausal women in Nigeria.

There still exist a strong adherence to traditional and cultural practices in Nigeria where the way of life places a lot of demand on the women in the running of the household.

Aim: The aim of this study is to investigate the role of perceived family support and self-care practices in women of menopausal age group attending the General Outpatient (GOP) Clinic of Federal Medical Centre, Owo, with the purpose of improving the knowledge of menopause related problems and reducing potential health related problems within this age group.

Casebook for the award of fellowship:

A casebook of 20 patients managed for the award of the Fellowship of the College in the faculty of Family Medicine. A total of 173 pages of 20 cases were written.

These cases comprise of obstetric and gynecological cases, internal medicine, general surgery, orthopedics, pediatrics, ENT, psychiatry, ophthalmology and a family case study.

The purpose of the casebook is to demonstrate competencies in the art of history taking, appropriate physical examination, cost-effective laboratory investigations, patient-centered care approach to managing each patient and discussion of each case with references to literature.

The continual demonstration of evidence base medicine in the management of each case exemplifies biopsychosocial wellbeing.
