Provide home health visits and admissions per Medicare guidelines. Perform and oversee chronic illness teaching and administration of skilled care in order to prevent re-hospitalization and promote safety and wellness in the home.
Conduct the assessment of systems and assessed for complications of therapy. Implement and oversee for the efficacy of treatment modalities. Confer with my team through team meetings with the multidisciplinary team to promote communication and continuity of care.
