OBJECTIVE

My ideal position would be to work in an environment where I can collaborate with other health care professionals to provide excellent care to patients, grow in my clinical skills and use my experience to improve access to care. While my primary experience is in family practice, I enjoy using my skills in orthopedics, women’s care, pediatrics and would like to utilize my experience in a new setting.

PROFESSIONAL EXPERIENCE

2005-present, UPMC Hamot Medical Center, Heritage Primary Care (Waterford, PA). Primary care provider in a suburban family practice with three physicians and one other PA-C. Responsible for diverse patient population from newborn to geriatric care. Patient care involving detailed patient history and physical exams, formulating treatment plans, prescribing medications, ordering and interpreting diagnostic tests, small in-office procedures, gynecological care. Trained in Allscripts electronic health records and used from 2007-2012. Trained and currently using EpicCare, electronic health records and billing. Also responsible for sharing call on a rotation with other providers in the practice. Part of pilot program and certified practice for Patient Centered Medical Home for diabetes and smoking cessation.

2009-present, Preceptor to Gannon and Edinboro University APP students in our office for their family practice rotations. Provide instructing, evaluation and teaching to students, co-signing charts.

2005-present, General McLane HS, Fort LeBeouf School Districts. Employed 1-2 times throughout the year to the school districts to complete sports and school physicals to middle and high school.

2005-2007, Hamot Medical Center, Family Health Care of Edinboro (Edinboro, PA). Provider part-time in a Hamot family practice until all hours were officially allotted to Heritage Primary Care.

2003-2005, Middlesex Gastroenterology Associates (Middletown, CT). Provider employed in a large private specialty group of six gastroenterologists and one other physician assistant. Responsible for seeing consults and follow-ups in the office, patient education, completing history and physical examinations in the hospital and office setting, in-patient care management under the gastroenterologist’s service.

State Physician Assistant License. Pennsylvania 2004-present. Connecticut 2003-2005.

Advanced Cardiac Life Support. Certified in 2002. Recertified in 2006, 2008, 2011, 2013 and 2015.

Basic Life Support certification. Certifications kept up to date since 1998. Most recently June 2015.

Pennsylvania Society of Physician’s Assistants: Member since 2011.

EDUCATION

Clinical rotations included: family practice, pediatrics, internal medicine, OB/GYN, general/vascular/orthopedic surgery, psychiatry and elective in neonatal intensive care in Harrisburg, PA.

1996-2001, Allied Health Sciences, Slippery Rock University, Slippery Rock, PA. Bachelor of Science in Allied Health Science with concentration in athletic training. Maintained NATA certification until 2003.

VOLUNTEER EXPERIENCE

Emma’s Footprints: Serve as a volunteer in ministry serving families suffering pregnancy and infant loss.
