Job description:
Responsible for day to day care of patients in a Skilled Nursing Facility as a Clinician, Educator and in a Leadership role.
Manage acute and chronic illnesses that may stand alone or associated with Alzheimers or Dementia, Diabetes Mellitus, Coronary Artery Disease, chronic Kidney disease, Chronic Obstructive Pulmonary Disease and Palliative Care.
Coordinate services provided by multiple disciplines in the community. Provide educational support to patients and their families, health team members and the community.
Manage transition to home care with focus on reducing re-hospitalizations.
